目的探讨滑膜肉瘤中上皮钙黏素(E-cad)和β-连接素(β-cat)的表达特征及意义。方法采用组织芯片技术和免疫组织化学方法,检测72例滑膜肉瘤组织中E-cad和β-cat的表达情况,并分析其与临床病理及生存率的关系。结果(1)组织芯片免疫组化染色后,形态可观测率为95.1%,并且背景清晰,对比鲜明。(2)在72例患者中,E-cad低表达者56例,占77.8%;β-cat低表达者51例,占70.8%。(3)单相梭形细胞为主型、组织学Ⅲ级、复发转移患者、细胞角蛋白(CK)阴性组和上皮膜抗原(EMA)阴性组的E-cad、β-cat低表达率显著高于双相分化型、组织学Ⅱ级、原发性患者、CK阳性组和EMA阳性组(P均<0.05)。(4)E-cad、β-cat正常表达组的生存情况明显优于低表达组(P均<0.05)。结论E-cad、β-cat的表达与滑膜肉瘤细胞分化过程有关,其低表达可能提示肿瘤复发转移潜能增强,患者预后不良。组织芯片适用于大样本回顾性研究。
Objective To investigate the expression and significance of E-cad and β-cat in synovial sarcoma. Methods Tissue microarray and immunohistochemistry were used to detect the expression of E-cad and β-cat in 72 cases of synovial sarcoma. The relationship between the expression of E-cad and β-cat was analyzed with the clinicopathological factors and survival rate. Results (1) Tissue microarray immunohistochemical staining, the morphology observed rate was 95.1%, and the background is clear, the contrast. (2) Among 72 patients, 56 cases were low expression of E-cad, accounting for 77.8%; 51 cases were low expression of β-cat, accounting for 70.8%. (3) The single-phase spindle cells, histological grade Ⅲ, recurrence and metastasis, cytokeratin (CK) negative group and epithelial membrane antigen (EMA) negative group of E-cad, β-cat low expression was significant Higher than bipolar differentiation, histological grade Ⅱ, primary patients, CK positive group and EMA positive group (all P <0.05). (4) The survival of normal E-cad and β-cat expression group was significantly better than that of low expression group (all P <0.05). Conclusions The expression of E-cad and β-cat is related to the differentiation of synovial sarcoma cells. The low expression of E-cad and β-cat may suggest that the potential of tumor recurrence and metastasis is enhanced and the prognosis is poor. Tissue chips are suitable for large sample retrospective studies.
