目的了解广州市萝岗区区属医院、村卫生站、民营医院、私营诊所等不同类型医疗机构的消毒工作质量,比较是否存在差距,并探讨消毒中存在的问题。方法对萝岗区不同类型医疗机构的手术室空气、物体表面与医护人员手、使用中消毒液、无菌医疗用品、处理后医疗废水、压力蒸汽灭菌器和使用中紫外线灯进行采样监测。结果4年共检测样品1 866份,合格1 647份,平均总合格率为88.26%。2009-2012年合格率分别为84.94%、82.92%、93.50%和89.66%。不同监测项目中,合格率较高的是压力蒸汽灭菌器(100.00%)和使用中消毒液(98.72%),合格率较低的是处理后医疗废水(54.05%)。区属村卫生站合格率高于私营诊所,差异有统计学意义(P<0.01)。结论在消毒工作质量方面区属医院和民营院水平相当,而私营诊所与区属村卫生站存在一定差距,需要对私营诊所加强院感预防控制管理、技术知识培训,提高其院感控制能力。
Objective To understand the quality of disinfection work in different types of medical institutions such as district hospitals, village health stations, private hospitals and private clinics in Luogang District of Guangzhou City, and to find out the gaps between the two types and to discuss the problems in disinfection. Methods Sampling and monitoring were performed on the operating room air, the surface of the operating room, the hands of medical staff, the disinfectant used, the sterile medical supplies, the treated medical waste water, the pressure steam sterilizer and the ultraviolet lamp in operation in different types of medical institutions in Luogang District. Results A total of 1 866 samples were tested in 4 years, with a total of 1 647 samples. The average passing rate was 88.26%. The passing rates in 2009-2012 are 84.94%, 82.92%, 93.50% and 89.66% respectively. Among the different monitoring items, the higher pass rate is the pressure steam sterilizer (100.00%) and the disinfectant in use (98.72%), and the lower pass rate is the treated medical wastewater (54.05%). The qualified rate of the district health station was higher than that of the private clinic, the difference was statistically significant (P <0.01). Conclusion There is a considerable gap between the quality of disinfection work in hospitals and privately owned hospitals. However, there is a certain gap between private clinics and village clinics. It is necessary to strengthen the training of prevention and control of nosocomial infections and technical knowledge in private clinics and to improve their ability to control nosocomial infections.
