目的观察高血压合并糖尿病时加重脂质代谢紊乱的特点。方法将高血压或(和)糖尿病患者86例,分为单纯高血压组65例,高血压合并糖尿病组21例。观察脂蛋白(a)(LPA)、载脂蛋白AI(APOAL)、载脂蛋白B(APOB)、胆固醇(TC)、甘油三脂(TG)、高密度脂蛋白(HDLC)、低密度脂蛋白(LDLC)和极低密度脂蛋白(VLDLC)的数值。结果高血压合并糖尿病组患者脂蛋白(a)水平较单纯高血压组患者高,统计学有显著性差异(P=0.019,P<0.05)。高血压合并糖尿病组患者甘油三脂及极低密度脂蛋白水平均较单纯高血压组患者有非常明显增高,统计学有极显著性差异(P=0.002,P<0.01)。结论高血压合并糖尿病时脂蛋白(a)、甘油三脂和极低密度脂蛋白的紊乱具有叠加作用。因此,在积极控制血压的同时要积极控制血糖和血脂,以更加有效地预防心脑血管疾病的发生。
Objective To observe the characteristics of aggravating dyslipidemia in hypertensive patients with diabetes mellitus. Methods Eighty-six patients with hypertension or (or) diabetes mellitus were divided into simple hypertension group (n = 65) and hypertension group (n = 21). (LPA), apolipoprotein AI (APOAL), APOB, TC, TG, HDLC, low density lipoprotein (LDLC) and very low density lipoprotein (VLDLC) values. Results The level of lipoprotein (a) in hypertensive patients with diabetes mellitus was significantly higher than that in patients with hypertension alone (P = 0.019, P <0.05). The levels of triglyceride and very-low density lipoprotein in hypertensive patients with diabetes mellitus were significantly higher than those in patients with hypertension alone. There was a statistically significant difference (P = 0.002, P <0.01). Conclusions The disorder of lipoprotein (a), triglyceride and very low density lipoprotein in hypertension with diabetes mellitus has superposition effect. Therefore, in the active control of blood pressure at the same time to actively control blood sugar and blood lipids, in order to more effectively prevent the occurrence of cardiovascular and cerebrovascular diseases.
