微波、热活检及高频电切治疗食管乳头状瘤67例疗效观察及随访

目的探讨内镜下微波凝固、热活检及高频电切治疗食管乳头状瘤的有效性及安全性。方法内镜下对67例食管乳头状瘤患者按乳头状瘤大小分别行微波凝固、热活检钳电凝、圈套器电切治疗,术后口服奥美拉唑胶囊20 mg,2次/d,服用14 d,术后1、3、12个月随访并复查胃镜,观察瘤体脱落及复发情况。结果 67例均一次成功完成内镜下治疗,术中无并发症,术后24例胸骨后灼痛,48 h后消失,无需特殊处理;术后1个月复查胃镜乳头状瘤全部脱落,3个月后复查胃镜,局部黏膜修复正常,12个月后61例复查无复发,6例失访。结论微波凝固、热活检及高频电切治疗食管乳头状瘤安全有效,无复发。 Objective To investigate the efficacy and safety of endoscopic microwave coagulation, thermal biopsy and high-frequency electrotomy in the treatment of esophageal papilloma. Methods Sixty-seven patients with esophageal papilloma were treated with microwave coagulation, biopsy forceps and electrosurgical forceps respectively according to the size of papilloma. The patients were treated with omeprazole 20 mg twice daily, After 14 days of operation, follow-up was performed at 1, 3, and 12 months after operation and gastroscopy was performed. The tumor shedding and recurrence were observed. Results 67 cases were successfully completed endoscopic treatment, no intraoperative complications, postoperative 24 cases of sternum after burning pain disappeared after 48 h, without special treatment; 1 month after the endoscopy all the endoscopic papilloma, 3 Gastroscopy was performed after a month and local mucosal repair was normal. No recurrence was found in 61 cases after 12 months and 6 cases were lost to follow-up. Conclusion Microwave coagulation, thermal biopsy and high-frequency electrosurgical treatment of esophageal papilloma safe and effective, no recurrence.
