目的:观察天麻钩藤饮对风阳上扰型急性脑梗死患者血脂、血清过氧化氢酶(CAT)水平的影响。方法:选择风阳上扰型急性脑梗死患者68例,随机分为治疗组和对照组各34例,对照组给予脑梗死常规治疗(口服拜阿司匹林100 mg,qd和立普妥20 mg,qn),治疗组在脑梗死常规治疗基础上加服天麻钩藤饮汤剂,以7 d为1个疗程,共治疗2个疗程。观察治疗前后血脂水平,CAT水平变化,测定治疗前后神经功能缺损评分及中医证候积分的变化。结果:与对照组比较,治疗组总胆固醇(TC),甘油三酯(TG),低密度脂蛋白胆固醇(LDL-C)和载脂蛋白-B(Apo-B)水平均降低,分别为(4.08±0.67),(1.20±0.65),(1.98±0.24)mmol·L~(-1),(1.03±0.41)g·L~(-1),而CAT水平明显升高(20.07±5.83)U·L~(-1),两组治疗后比较有统计学意义(P<0.05,P<0.01);治疗组神经功能缺损评分和中医证候积分均降低,分别为(3.65±2.15)分,(9.27±7.11)分,两组治疗后比较有统计学意义(P<0.05)。治疗组总有效率为88.2%,对照组总有效率为79.4%,治疗组临床疗效更为显著(P<0.05)。结论:天麻钩藤饮可以有效的降低风阳上扰型急性脑梗死的血脂水平、升高CAT水平,从而提高患者抗氧化能力。
Objective: To observe the effect of Tianma Gouteng Decoction on serum lipid and serum catalase (CAT) in patients with wind-induced disturbance of acute cerebral infarction. Methods: A total of 68 patients with wind-induced disturbance of acute cerebral infarction were randomly divided into treatment group (34 cases) and control group (34 cases). The control group was given routine treatment of cerebral infarction (oral aspirin 100 mg, qd and Lipitor 20 mg, qn ). The treatment group was given Tianma Gouteng Decoction on the basis of conventional treatment of cerebral infarction, taking 7 days as a course of treatment for a total of 2 courses of treatment. Blood lipid levels and CAT levels were observed before and after treatment. Changes in neurological deficit scores and TCM syndrome scores were determined before and after treatment. Results: Compared with the control group, the levels of total cholesterol (TC), triglyceride (TG), low density lipoprotein cholesterol (LDL-C) and apolipoprotein B 4.08 ± 0.67), (1.20 ± 0.65), (1.98 ± 0.24) mmol·L -1, (1.03 ± 0.41) g · L -1, while the level of CAT increased significantly (20.07 ± 5.83) (P <0.05, P <0.01). The scores of neurological deficit and TCM syndromes in treatment group were significantly lower than those in control group (3.65 ± 2.15) , (9.27 ± 7.11) points, the two groups after treatment were statistically significant (P <0.05). The total effective rate was 88.2% in the treatment group and 79.4% in the control group, and the clinical effect was more significant in the treatment group (P <0.05). Conclusion: Tianma Gouteng Decoction can effectively reduce the level of blood lipid and increase the level of CAT in the patients with acute cerebral infarction complicated with wind-yang disturbance, thereby enhancing the anti-oxidation ability of patients.
