目的调查本地腹泻病人中小肠结肠炎耶尔森菌的感染现状,开展小肠结肠炎耶尔森菌的病原学研究,为制定相应的防治对策提供科学依据。方法参照中国CDC编《小肠结肠炎耶尔森菌的分离与鉴定手册》制定的检验程序,开展腹泻病人标本病原检测和微生物学研究。结果 2 193份腹泻病人标本中检出小肠结肠炎耶尔森菌40株。毒力基因检测其中O3型菌株的基因分布为ail+、yst A+、yad A+、vir F+、yst B-;PFGE分子分型显示腹泻病人和宿主动物的O3型菌株相似度为96.0%~100.0%。结论本地腹泻病人标本中检出了小肠结肠炎耶尔森菌,冬春季该菌的阳性检出率显著升高;O3型菌株均携带有上述毒力基因,具有典型的致病性特征;PFGE分子分型检测表明腹泻病人O3型菌株感染与宿主动物之间有密切的内在联系。
Objective To investigate the status of Yersinia enterocolitica infection in local diarrhea patients and carry out the etiological study of Yersinia enterocolitica in order to provide a scientific basis for the development of appropriate control strategies. Methods According to the test program developed by CDC, “Handbook of Isolation and Identification of Yersinia enterocolitica”, the pathogen detection and microbiology of patients with diarrhea were carried out. Results A total of 40 strains of Yersinia enterocolitica were detected in 2 193 diarrhea patients. The virulence genes were detected by the genotypes O3. The genotypes of O3 strains were ail +, yst A +, yad A +, vir F +, yst B-. The PFGE molecular typing showed that the similarity of O3-type strains between diarrhea patients and host animals was 96.0% -100.0%. Conclusion The positive rate of Yersinia enterocolitica was detected in the specimens from local diarrhea patients. The positive rate of Yersinia enterocolitica was significantly increased in winter and spring. The O3 strains harboring the above virulence genes have typical pathogenicity. PFGE Molecular typing tests show that there is a close relationship between O3-type infection and host animals in diarrhea patients.
