目的:探讨大鼠充血性心力衰竭心肌间质基质金属蛋白酶-2(MMP-2)的表达和心功能变化的关系及厄贝沙坦干预对此影响。方法:选用90只SD大鼠,8只不结扎冠状动脉列为假手术组(S组);其余82只结扎其左冠状动脉主干制成心肌梗死模型后,再分为对照组(C组)和厄贝沙干预组(E组)。应用超声心动图评价各组大鼠心功能变化,用免疫组化及Western免疫印迹方法检测各组大鼠MMP-2蛋白表达情况。结果:与S组相比,C组MMP-2的蛋白表达在实验第2周、4周、8周3个时点均明显升高(均P<0.05),而E组MMP-2蛋白表达在第4周和第8周明显降低(均P<0.05)。超声心动图结果显示C组、E组大鼠左室结构和功能呈现动态变化,心功能进行性降低,与C组相比,E组第8周心功能各项指标均有不同程度的改善。结论:大鼠心肌梗死后心肌间质内MMP-2的表达增高,是心室重塑和心力衰竭发生发展的机制之一。厄贝沙坦可通过抑制MMP-2的表达而改善心室重塑及心功能。
Objective: To investigate the relationship between the expression of matrix metalloproteinase-2 (MMP-2) and cardiac function in rats with congestive heart failure and the influence of irbesartan intervention on it. Methods: Ninety Sprague-Dawley rats were randomly divided into sham operation group (n = 8) and non-ligation of coronary artery (n = 8). The remaining 82 rats were ligated into the left coronary artery to establish myocardial infarction model, And irbesa intervention group (group E). The changes of cardiac function in rats in each group were evaluated by echocardiography. The expression of MMP-2 protein in each group was detected by immunohistochemistry and Western blotting. Results: Compared with S group, the protein expression of MMP-2 in group C was significantly increased at 3, 4, and 8 weeks (all P <0.05), while the expression of MMP-2 protein in group E Decreased significantly at week 4 and week 8 (all P <0.05). The results of echocardiography showed that the left ventricular structure and function of group C and group E presented a dynamic change and the heart function progressively decreased. Compared with group C, the indexes of cardiac function in group E at the eighth week all improved to some extent. Conclusion: The increased expression of MMP-2 in myocardial interstitium after myocardial infarction in rats is one of the mechanisms of ventricular remodeling and heart failure. Irbesartan can improve ventricular remodeling and cardiac function by inhibiting the expression of MMP-2.
