目的:探讨MMP28在膀胱癌中的表达情况及其与临床分期、组织学分级、预后等指标的关系,分析MMP28表达对膀胱癌预后的影响。方法:收集8对新鲜的膀胱癌组织及其配对的正常膀胱组织和173例膀胱癌组织蜡块,分别采用qRT-PCR和免疫组化法检测MMP28的表达,结合相关病理和随访指标,采用χ2检验、Kaplan-Meier法、Cox回归进行统计分析。结果:在转录水平上,膀胱癌组织的MMP28表达高于正常的膀胱癌组织,有统计学意义。MMP28蛋白在膀胱癌中的表达与其年龄、病理分级、临床分期有关(P=0.039,P=0.024,P=0.003),与性别、肿瘤数量、肿瘤直径无关(P=0.120,P=0.266,P=0.462)。膀胱癌中MMP28蛋白高表达组的总体生存率和无进展生存率显著低于低表达组(P=0.002,P=0.001)。单因素分析发现影响膀胱癌患者预后的因素有患者的年龄(P=0.006,P=0.007)、病理学分级(P=0.008,P=0.006)、肿瘤数目(P=0.012,P=0.013)、临床分期(P=0.003,P=0.003)和MMP28蛋白的表达(P=0.002,P=0.001)。而且,多因素分析表明只有临床分期(P=0.022,P=0.043)、MMP28(P=0.037,P=0.045)是预测膀胱癌总体生存和无进展生存的独立因素。结论:MMP28在膀胱癌中的表达是明显上调的。MMP28表达与膀胱癌的年龄较大、高组织分级、较晚临床分期、预后差呈正相关,是预测膀胱癌生存的一个独立因素,可能成为膀胱癌治疗的一个新靶点。
Objective: To investigate the expression of MMP28 in bladder cancer and its relationship with clinical stage, histological grade, prognosis and other indicators of MMP28 expression on the prognosis of bladder cancer. Methods: Eight pairs of fresh bladder cancer tissues, paired normal bladder tissues and 173 cases of bladder cancer tissue paraffin blocks were collected. The expression of MMP28 was detected by qRT-PCR and immunohistochemistry. Combined with related pathology and follow-up indicators, Test, Kaplan-Meier method, Cox regression for statistical analysis. Results: At the transcriptional level, the expression of MMP28 in bladder cancer tissues was higher than that in normal bladder cancer tissues, with statistical significance. The expression of MMP28 protein in bladder cancer was related to age, pathological grade and clinical stage (P = 0.039, P = 0.024, P = 0.003), but not to sex, tumor number and tumor diameter = 0.462). The overall survival and progression-free survival of patients with high MMP28 expression in bladder cancer were significantly lower than those in the low expression group (P = 0.002, P = 0.001). Univariate analysis showed that the prognostic factors of patients with bladder cancer were age (P = 0.006, P = 0.007), pathological grade (P = 0.008, P = 0.006), tumor number (P = 0.012, P = 0.013) Clinical stage (P = 0.003, P = 0.003) and MMP28 protein expression (P = 0.002, P = 0.001). Moreover, multivariate analysis showed that only clinical stage (P = 0.022, P = 0.043) and MMP28 (P = 0.037, P = 0.045) were independent predictors of overall survival and progression-free survival in bladder cancer. Conclusion: The expression of MMP28 in bladder cancer is significantly up-regulated. The expression of MMP28 is positively correlated with the age, histological grade, clinical stage and prognosis of bladder cancer, which is an independent factor in predicting the survival of bladder cancer and may be a new target for the treatment of bladder cancer.
