目的 探讨支气管哮喘患者气道粘膜上皮细胞是否表达基质金属蛋白酶 9(MMP 9)及其与气道嗜酸粒细胞浸润、气流受限的关系。方法 选取 10例支气管哮喘患者、6例COPD患者及 10例正常对照 ,进行纤维支气管镜粘膜活检 ,对活检组织做MMP 9免疫组化及HE染色 ;同时对患者进行肺功能测定及组胺激发实验。结果 所有支气管哮喘患者气道粘膜上皮MMP 9免疫组化均为阳性 ,2 / 6的COPD患者气道粘膜上皮MMP 9免疫组化为阳性 ,而所有正常对照气道粘膜上皮MMP 9免疫组化均为阴性 ;相关分析显示 :支气管哮喘患者气道粘膜上皮MMP 9免疫组化评分与气道嗜酸粒细胞计数呈正相关 (r =0 6 2 ,P <0 0 1) ,与FEV1呈负相关 (r =- 0 5 1,P<0 0 1) ,与PD2 0 FEV1不相关。结论 支气管哮喘患者气道粘膜上皮表达MMP 9,此可能为导致气道嗜酸粒细胞浸润及气流受限的重要原因。
Objective To investigate whether matrix metalloproteinase 9 (MMP 9) is expressed in airway epithelial cells of bronchial asthma and its relationship with airway eosinophil infiltration and airflow limitation. Methods Ten patients with bronchial asthma, six patients with COPD and ten normal controls were selected for biopsy of fibrobronchial mucosa. Immunohistochemistry and HE staining were performed on the biopsy tissues. Pulmonary function test and histamine challenge test . Results All patients with bronchial asthma had positive MMP 9 immunohistochemistry in airway epithelium, positive MMP 9 expression in airway epithelium in 2 of 6 COPD patients, and all the normal control airway epithelium MMP 9 immunohistochemistry (R = 0.62, P <0.01), and was negatively correlated with FEV1 (P <0.01) .Conclusions: Bronchial asthma patients with bronchial asthma had a significant positive correlation with the number of eosinophils r = - 0 5 1, P <0 0 1), not related to PD2 0 FEV1. Conclusion The expression of MMP-9 in airway epithelium of patients with bronchial asthma may play an important role in the airway eosinophil infiltration and airflow limitation.
