目的探讨血清同型半胱氨酸(Hcy)、胱抑素C(Cys C)联合尿微量清蛋白(U-m ALB)检测于早期诊断糖尿病肾病(DN)中的临床价值。方法选择濮阳市第三人民医院46例DN患者为观察组,并选取同期健康体检者45例作对照组,检测比较两组血清Hcy、Cys C及U-m ALB水平,并观察其诊断DN的特异性、敏感性、符合率。结果观察组血清Hcy、Cys C及U-m ALB水平均高于对照组,差异有统计学意义(P<0.05);Hcy诊断DN特异性、敏感性、符合率分别为100.00%、69.57%、84.62%;Cys C分别为100.00%、71.74%、85.71%;U-m ALB分别为95.56%、73.91%、84.62%;联合检测分别为95.56%、93.48%、94.51%;联合检测诊断敏感性、符合率均高于单一指标检测,差异有统计学意义(P<0.05)。结论血清Hcy、Cys C及U-m ALB均与糖尿病肾病密切相关,应用三者联合检测有助于提高诊断符合率。
Objective To investigate the clinical value of serum homocysteine (Hcy), cystatin C (Cys C) combined with urinary microalbuminuria (U-m ALB) in the early diagnosis of diabetic nephropathy (DN). Methods 46 patients with DN in Puyang Third People’s Hospital were selected as the observation group and 45 healthy subjects were selected as the control group. The levels of Hcy, Cys C and Um ALB in the two groups were detected and compared, and the specificity of DN diagnosis Sensitivity, in line with the rate. Results The serum levels of Hcy, Cys C and Um ALB in the observation group were significantly higher than those in the control group (P <0.05). The specificity and sensitivity of Hcy in diagnosis of DN were 100.00%, 69.57% and 84.62% ; Cys C were 100.00%, 71.74% and 85.71% respectively; Um ALB were 95.56%, 73.91% and 84.62% respectively; the combined detection was 95.56%, 93.48% and 94.51% In a single index test, the difference was statistically significant (P <0.05). Conclusion Serum Hcy, Cys C and U-m ALB are closely related to diabetic nephropathy. The combination of these three tests may be helpful to improve the diagnostic accuracy.
