Ptf-V_1异常一般认为是由于左房扩大引起,近来有报道肺心病可出现异常的Ptf-V_1.肺心病主要为累及右心的病变,其出现异常的Ptf-V_1是否也是由于左房扩大所致,值得进一步研究.本文将肺心病人出现Ptf-V_1时的超声心动图测定左房内径,并以风湿性二尖瓣病变作对比研究,发现肺心病11例,Ptf-V_1平均值为-0.034±0.009mms,左房内径平均值为23.4±4.4mm(正常值为28.0±4.0mm): 而风心病10例,Ptf-V_1平均值为0.048±0.30mms,左内径为45.0±5.8mm,肺心病人Ptf-V_1大多在-0.039mms以上,数值大小与左房内径无关;风心Ptf-V_1大多在-0.03mms以下,数值大小与左房内径呈相应的改变.因此,肺心病人出现Ptf-V_1并非为左房扩大所致,可能与扩大的右房向后移位而反映在水平轴上为V_1P波终末向量或房内阻滞有关.
