目的:分析接受辅助生殖技术(ART)出现异位妊娠的类型以及高危因素。方法:随机选取2015年1月至2016年9月东莞市第三人民医院接诊的22例行ART后并发异位妊娠的患者,同期选择22例行ART后未出现异位妊娠的孕妇作为对照组,统计分析行ART后并发异位妊娠的类型,并对异位妊娠高危因素进行回顾性分析,为临床诊断和治疗提高科学依据。结果:所有行ART后并发异位妊娠的22例孕妇中,均为输卵管妊娠;观察组继发性不孕、既往行人工流产、支原体感染史、输卵管手术史以及输卵管发生阻塞的比例均显著高于对照组,差异具有统计学意义(P<0.05);两组患者在人绒毛膜促性腺激素(HCG)日的激素指标水平差异无统计学意义(P>0.05);观察组患者HCG日子宫内膜厚度显著低于对照组,差异具有统计学意义(P<0.05)。结论:不孕类型、既往行人工流产、支原体感染史、输卵管手术史、输卵管阻塞以及HCG日子宫内膜较薄是导致ART后出现异位妊娠的主要危险因素,在临床中应给予足够关注,并积极采取有效措施,以保障母婴安全。
OBJECTIVE: To analyze the types of ectopic pregnancy and the risk factors for receiving ART. Methods: Twenty-two patients with ectopic pregnancy after ART were selected randomly from January 2015 to September 2016 in Dongguan Third People’s Hospital. Twenty-two pregnant women who did not have ectopic pregnancy after ART were selected as control Group, statistical analysis of the type of ectopic pregnancy complicated by ART, and retrospective analysis of risk factors for ectopic pregnancy, to improve the scientific basis for clinical diagnosis and treatment. Results: All the 22 pregnant women who had ectopic pregnancy after ART were tubal pregnancy. The rates of secondary infertility, history of induced abortion, history of mycoplasma infection, history of tubal surgery and tubal occlusion in observation group were significantly higher (P <0.05). There was no significant difference in the level of hormones between the two groups on the day of human chorionic gonadotropin (HCG) (P> 0.05); on the day of HCG day in the observation group, the difference was statistically significant Intimal thickness was significantly lower than the control group, the difference was statistically significant (P <0.05). CONCLUSIONS: Infertility type, history of induced abortion, history of mycoplasma infection, history of tubal surgery, fallopian tube obstruction and endometrial thinning at HCG are the major risk factors for ectopic pregnancy after ART. Adequate attention should be paid in clinical practice, And actively take effective measures to protect the safety of mothers and children.
