目的调查分析2014-2015年衢州地区洗涤红细胞的临床应用情况,为临床合理、有效地应用洗涤红细胞提供依据。方法回顾性分析2014-2015年衢州地区8家二级及以上医院洗涤红细胞的用量、输注病例的疾病分布、输注前血清学检测等情况。结果 2015年衢州地区洗涤红细胞用量比2014年增长了39.07%,增幅是红细胞类用量增幅的24.4倍,增幅主要集中3医院;血液病、恶性肿瘤和慢性肾病是输注病例的主要诊断,分别占输注病例的63.49%、19.05%、12.70%;输注前血清学检测项目各家用血医院开展情况标准不一。结论洗涤红细胞的优点逐步被临床医生所认识,但也切不可经验性应用,用血医院应重视临床输血前综合评估和规范输血科血清学检测是提高该成分性价比的关键。
Objective To investigate and analyze the clinical application of washing red blood cells in Quzhou from 2014 to 2015 so as to provide evidences for the rational and effective use of washing red blood cells. Methods Retrospective analysis was conducted on the amount of washed red blood cells, the distribution of disease in infusion cases and the serological test before infusion in 8 hospitals of secondary level II and above in Quzhou from 2014 to 2015. Results In 2015, the amount of washed red blood cells in Quzhou increased by 39.07% compared with that in 2014, an increase of 24.4 times that of erythrocyte. The increase mainly concentrated in 3 hospitals. Hematopathy, malignant tumor and chronic kidney disease were the main diagnoses of infusion cases, accounting for 63.49%, 19.05% and 12.70% of the infused cases, respectively. The standards for the development of various blood-serum hospitals before the infusion test were different. Conclusion The advantages of washing erythrocytes are gradually recognized by clinicians, but they should not be used empirically. It is crucial for hospitals to pay attention to the comprehensive evaluation before clinical transfusion and to standardize the serological test of transfusion department to improve the cost-effectiveness of this component.
