酶转换法与高效液相色谱法检测急性心肌梗死患者血清HCY的准确性比较

目的探讨酶转换法与高效液相色谱法检测急性心肌梗死(AMI)患者血清同型半胱氨酸(HCY)的准确性。方法选取河南省电力医院2013年8月—2014年8月57例AMI患者为观察组,另选同期健康体检者50例为正常组,两组均利用酶转换法与高效液相色谱法两种方法检测HCY水平,比较两种检测方式重复性精密度、HCY阳性检出率及两种检测方式对观察组、正常组HCY水平的检测结果,从而分析两种检测技术的灵敏度与特异度。选用SPSS19.0统计学软件,计量资料用均数±标准差(x±s)表示,采用t检验,计数资料用率(%)表示,采用χ2检验,P<0.05表示差异有统计学意义。结果两种诊断方式重复性精密度试验显示,批间、批内CV<2%,总CV均值<3%;观察组酶转化法检测HCY阳性率为89.47%,高效液相色谱检测阳性率为94.74%,差异无统计学意义(P>0.05),正常组酶转化法检测1例HCY阳性,高效液相色谱法检测无阳性,差异无统计学意义(P>0.05);酶转换法诊断灵敏度与特异度分别为89.47%、98.00%,高效液相色谱法分别为94.74%、100.00%,差异均无统计学意义(P>0.05)。结论酶转换法与高效液相色谱法检测AMI患者血清HCY均有较高的准确性,且两种检测方式一致性较高,高效液相色谱法检测AMI患者HCY阳性的特异性相对较高,但仍需今后更大样本量的研究以进一步证实。 Objective To investigate the accuracy of enzymatic conversion and high performance liquid chromatography (HPLC) in detecting serum homocysteine ​​(HCY) in patients with acute myocardial infarction (AMI). Methods Fifty-seven patients with AMI in Henan Provincial Electric Power Hospital from August 2013 to August 2014 were selected as the observation group, and 50 healthy subjects in the same period were selected as the normal group. Results The reproducibility of the two diagnostic methods showed that CV <2%, total CV mean <3% between batches and batches, positive rate of HCY detected by enzymatic conversion assay was 89.47%, the positive rate of HPLC detection was 94.74%, the difference was not statistically significant (P> 0.05), normal group enzymatic conversion method was detected in 1 case of HCY positive, high-performance liquid chromatography did not detect positive, the difference was not statistically significant (P> 0.05); enzyme conversion method diagnostic sensitivity The specificity and specificity were 89.47%, 98.00%, respectively. There was no significant difference between them by HPLC (94.74%, 100.00%) (P> 0.05). Conclusions Both enzymatic conversion method and high performance liquid chromatography (HPLC) have higher accuracy in detecting serum HCY in patients with AMI, and the consistency of the two detection methods is higher. The specificity of HCY detection by AMI in patients with AMI is relatively high, But still need a larger sample of future research to further confirm.
