目的探讨肝癌衍生生长因子(HDGF)和微血管密度(MVD)在肝外胆管癌的表达及其与临床病理特征和预后的关系。方法采用链霉亲和素-过氧化物酶免疫组织化学法检测65例肝外胆管癌患者术后组织中HDGF和MVD的表达情况,分析其与临床病理特征以及患者预后的关系。结果免疫组化结果显示,HDGF和MVD在胆管癌组织中的阳性表达率分别为46.2%和58.5%;Spearman等级相关分析显示,HDGF和MVD有正相关关系(r=0.279,P=0.024);HDGF表达阳性患者的总生存率明显小于阴性患者的总生存率(23.3%vs 62.9%,P<0.001);多因素分析结果表明HDGF是一个独立的预后因素。结论 HDGF在肝外胆管癌中表达与MVD相关,可作为一个独立的预后指标,HDGF可能通过促进新生血管形成,在肝外胆管癌的发生发展中起重要作用。
Objective To investigate the expression of hepatocellular carcinoma-derived growth factor (HDGF) and microvessel density (MVD) in extrahepatic cholangiocarcinoma and its relationship with clinicopathological features and prognosis. Methods The expression of HDGF and MVD in 65 patients with extrahepatic cholangiocarcinoma was detected by streptavidin - peroxidase immunohistochemistry. The relationship between the expression of HDGF and MVD was analyzed with the clinicopathological characteristics and prognosis. Results The positive rates of HDGF and MVD in cholangiocarcinoma were 46.2% and 58.5% respectively by immunohistochemistry. Spearman rank correlation analysis showed that there was a positive correlation between HDGF and MVD (r = 0.279, P = 0.024). The overall survival rate of HDGF-positive patients was significantly lower than that of negative patients (23.3% vs 62.9%, P <0.001). Multivariate analysis showed that HDGF was an independent prognostic factor. Conclusions The expression of HDGF in extrahepatic cholangiocarcinoma is correlated with MVD and may serve as an independent prognostic marker. HDGF may play an important role in the pathogenesis of extrahepatic cholangiocarcinoma by promoting angiogenesis.
