宁波市老年2型糖尿病人骨密度与体成分血脂水平相关性分析

目的:了解宁波市老年2型糖尿病人群骨密度、体成分及血脂水平情况,探讨骨密度与体成分、血脂水平关系。方法:测定某社区卫生服务中心在档管理的60~69岁138名2型糖尿病患者的骨密度、体成分、握力以及血脂水平相关指标。结果:65~69年龄组握力显著性低于60~64年龄组(P<0.05),但女性年龄组之间差异无统计学意义。男女性别各年龄段体重、身高、BMI、腰围、肌肉重、脂肪重、去脂体重和肌肉量比值差异无统计学意义。男性和女性的空腹血糖,高龄组与低龄组相比,差异有统计学意义(P<0.05)。而男女性别中HbAlC、血脂、骨密度T值各年龄组之间差异无统计学意义。老年2型糖尿病患者的骨密度与体质指数、腰围、肌肉量比值和HbAlC水平均呈正相关(P<0.05),但与握力、肌肉量、脂肪量、去脂体重计血脂水平无明显关系。结论:老年2型糖尿病患者BMI、腰围、肌肉量与骨密度密切相关,可以作为评价骨骼健康状况的间接参考指标。 OBJECTIVE: To understand the bone mineral density, body composition and blood lipid levels in elderly type 2 diabetic population in Ningbo and to explore the relationship between bone mineral density and body composition and blood lipid levels. Methods: BMD, body composition, grip strength and serum lipids were measured in 138 type 2 diabetic patients aged 60-69 years managed by a community health service center. Results: The grip strength of 65 ~ 69 age group was significantly lower than that of 60 ~ 64 age group (P <0.05), but there was no significant difference between female age groups. There was no significant difference in body weight, height, BMI, waist circumference, muscle mass, fat weight, body fat mass and muscle mass between men and women in all ages. The fasting blood glucose in males and females was significantly higher than that in the younger group (P <0.05). However, there was no significant difference between male and female in HbA1c, blood lipid and bone mineral density (T-value) among different age groups. Bone mineral density was positively correlated with body mass index, waist circumference, muscle mass ratio and HbA1C level in elderly patients with type 2 diabetes mellitus (P <0.05), but not with grip strength, muscle mass, fat mass, and lipoid weight. Conclusion: BMI, waist circumference and muscle mass are closely related to bone mineral density in elderly patients with type 2 diabetes mellitus, which can be used as an indirect reference index to evaluate the health status of bone.
