目的探讨EGR相关融合基因在前列腺癌的早期诊断和淋巴结转移预测中的效果。方法选择本院2008年1月—2016年8月前列腺首次穿刺诊断为前列腺上皮内瘤患者临床病例665例进行分析,根据本院早期建立的诊断阈值,将首次穿刺诊断为前列腺上皮内瘤患者分为两组,观察比较两组患者重复穿刺后的结果。计数资料比较采用χ~2检验,P<0.05为差异有统计学意义。结果 ERG相关融合基因阳性组患者中前列腺癌的占72.36%(254/351),而ERG相关融合基因阴性组患者发现是前列腺癌所占的比例仅为0.96%(3/314),两组比较差异具有统计学意义(χ~2=356.430,P<0.05);ERG相关融合基因阳性组中,92.02%(323/351)的患者诊断出患有前列腺癌是通过重复穿刺时发现;ERG相关融合基因阴性组中,79.30%(249/314)的患者在重复穿刺时发现;通过两组的对比发现,在后续发现前列腺癌的风险上差异具有统计学意义(χ~2=22.304,P<0.05)。在所有的前列腺上皮内瘤患者中发现有35.79%(238/665)的患者经过多次穿刺诊断为前列腺癌,其中有225例为阳性组患者,仅有13例为阴性组患者。结论 EGR相关融合基因对早期前列腺癌的发现和治疗有着很大的帮助,对后续预测淋巴结转移也有一定价值,在临床上应该更好地推广,具有良好的应用前景。
Objective To investigate the effect of EGR-related fusion gene in early diagnosis of prostate cancer and prediction of lymph node metastasis. Methods A total of 665 cases of prostatic intraepithelial neoplasia were selected from January 2008 to August 2016 in our hospital. According to the diagnostic threshold established in our hospital, the first puncture was diagnosed as prostate intraepithelial neoplasia For the two groups, observed and compared the results of repeated puncture of two groups of patients. Count data were compared using χ ~ 2 test, P <0.05 for the difference was statistically significant. Results The positive rate of prostate cancer was 72.36% (254/351) in ERG-associated fusion gene-positive patients, but only 0.96% (3/314) in patients with negative ERG-associated fusion gene. The difference was statistically significant (χ ~ 2 = 356.430, P <0.05). Of the ERG-associated fusion gene positive patients, 92.02% (323/351) of patients diagnosed with prostate cancer were found by repeated punctures; ERG- In the gene negative group, 79.30% (249/314) of the patients were found repeatedly puncture. The comparison of the two groups showed that the risk of subsequent prostate cancer was statistically significant (χ ~ 2 = 22.304, P <0.05 ). In all patients with prostate intraepithelial neoplasia, 35.79% (238/665) of the patients were diagnosed with prostate cancer by multiple punctures, of which 225 were positive and only 13 were negative. CONCLUSION: EGR-related fusion gene is of great help to the discovery and treatment of early-stage prostate cancer. It also has certain value for the subsequent prediction of lymph node metastasis. It should be better popularized in clinical practice and has good application prospect.
