牙齿由于龋坏、死髓、根管充填或塑化治疗等原因,咀嚼不慎而导致牙冠纵形裂开.以双尖牙和后磨牙为多见.以往常将裂牙拔除.现已普遍应用钢丝结扎,全冠修复后,恢复牙齿的功能.近几年来,钢丝结扎在我科已作为纵裂牙全冠修复前的常规.通过临床观察,我们发现,将纵裂牙直接用钢丝结扎后,钢丝易滑向牙颈部,导致结扎松动,还刺激牙龈(图1).针对这一情况,我们作了一点改进:将纵裂牙冠直径最大处的游离瓣颊面或舌(腭)面,用倒锥钻作一横槽(图2),给扎钢丝经此槽内结扎于冠部钢丝便不向牙颈部滑动,既不刺激牙龈,也获得满意的固定效果.
