目的探讨苯磺酸氨氯地平在糖尿病合并高血压患者治疗中,对血压晨峰现象的控制作用。方法 108例2型糖尿病合并高血压患者,随机分为观察组与对照组,各54例。两组均采用二甲双胍联合沙格列汀控制血糖至正常范围。其中观察组采用苯磺酸氨氯地平降压治疗,对照组采用硝苯地平缓释片治疗。监测两组治疗后3~4、7~8、15~16、30~31 d是否出现血压晨峰现象,并进行组间比较。结果两组患者治疗期间,血糖值未有明显波动,均控制在正常范围内。观察组治疗后第3~4、7~8、15~16、30~31天的血压晨峰已控制例数分别为7、18、30、46例,对照组分别为6、15、19、25例。观察组在治疗后第3~4天与第7~8天血压晨峰已控制例数均高于对照组,但比较差异无统计学意义(χ~2=0.087、0.393,P>0.05);治疗后第15~16天与第30~31天,观察组患者的血压晨峰已控制例数均高于对照组,差异具有统计学意义(χ~2=4.520、18.130,P<0.05)。结论苯磺酸氨氯地平对2型糖尿病合并高血压患者的血压晨峰现象控制良好。
Objective To investigate the control effect of amlodipine besylate on the morning peak of blood pressure during the treatment of diabetic patients with hypertension. Methods A total of 108 patients with type 2 diabetes mellitus complicated with hypertension were randomly divided into observation group and control group, with 54 cases in each group. Both metformin combined with saxagliptin controlled blood glucose to the normal range. The observation group was treated with amlodipine besylate and the control group was treated with nifedipine sustained-release tablets. The two groups were monitored after 3 ~ 4, 7 ~ 8, 15 ~ 16, 30 ~ 31 d whether there was a peak morning blood pressure phenomenon, and compared between groups. Results During the treatment of two groups, there was no obvious fluctuation of blood glucose level, all within the normal range. In the observation group, the controlled cases of morning blood pressure at 3 ~ 4, 7 ~ 8, 15 ~ 16 and 30 ~ 31 days after treatment were 7, 18, 30 and 46 cases respectively, while the control group were 6, 25 cases. In the observation group, the controlled cases of morning blood pressure at the third to fourth days and the seventh to eighth days after treatment were all higher than those in the control group, but the difference was not statistically significant (χ ~ 2 = 0.087, 0.393, P> 0.05). On the 15th to 16th and the 30th to 31st days after treatment, the controlled cases of morning blood pressure in the observation group were higher than those in the control group (χ ~ 2 = 4.520,18.130, P <0.05). Conclusion Amlodipine besylate has a good control of blood pressure morning peak in type 2 diabetic patients with hypertension.
