目的探讨超声引导下粗针活检组织病理检查诊断乳腺癌的价值及乳腺癌组织中人表皮生长因子受体-2(human epidemic growth factor receptor-2,HER-2)基因表达情况。方法 36例乳腺癌患者均行超声引导下乳腺肿块穿刺活检组织病理学检查,并采用荧光原位杂交(fluorescence in situ hybridization,FISH)方法检测HER-2表达情况。结果 36例取材满意率为100%,穿刺活检后无明显并发症发生;组织病理诊断浸润性癌26例,腺癌7例,导管原位癌1例,浸润性癌伴鳞状化生1例,大部分中-高级别导管原位癌+小部分浸润性癌Ⅱ~Ⅲ级1例;HER-2阳性18例,阴性16例,2例无法确定。结论超声引导下粗针穿刺组织病理活检明确乳腺癌诊断,可作为乳腺癌HER-2基因检测的取材方法。
Objective To investigate the value of ultrasound guided needle biopsy in the diagnosis of breast cancer and the expression of human epidermal growth factor receptor-2 (HER-2) gene in breast cancer. Methods Thirty - six patients with breast cancer underwent ultrasound - guided histological examination of breast biopsies. The expression of HER - 2 was detected by fluorescence in situ hybridization (FISH). Results The satisfaction rate of 36 cases was 100%. There was no obvious complication after biopsy. There were 26 cases of invasive carcinoma, 7 cases of adenocarcinoma, 1 case of ductal carcinoma in situ, 1 case of invasive carcinoma with squamous metaplasia , Most of the medium-high grade ductal carcinoma in situ + a small number of invasive carcinoma Ⅱ ~ Ⅲ grade in 1 case; HER-2 positive in 18 cases, negative in 16 cases, 2 cases can not be determined. Conclusion Ultrasound-guided needle biopsy histologically confirmed breast cancer diagnosis, HER-2 gene can be used as breast cancer screening methods.
