目的探讨快速双回波自旋序列(Pd+T2_tse)在立体定向丘脑毁损术治疗帕金森病中MR靶点定位的参数选择和临床应用价值。方法本院2008年1月~2010年1月行立体定向丘脑毁损术的33例帕金森病人(男25例,女8例),安装Leksell立体定向仪后,选择合适的参数扫描,然后导入Surgiplan立体定向计划系统(简称SPS),行术中脑深部微电极记录电生理验证后,进行丘脑Vim和Vo核的射频毁损,术后MR复查进行验证靶点的准确性。结果 33例病例MRI扫描均在同一层面显示AC、PC,基底节区各灰质团边界及其内囊、视束等毗邻结构显示清晰,MR图像资料皆顺利导入SPS手术计划系统。结论采用Pd+T2_tse序列,磁共振成像系统对靶点定位更加快速和精确。
Objective To investigate the parameter selection and clinical application of rapid double-echo spin sequence (Pd + T2_tse) in stereotactic thalamic damage in the treatment of Parkinson’s disease (MR). Methods From January 2008 to January 2010, 33 patients with Parkinson’s disease (25 males and 8 females) under stereotactic Thalamic rupture were enrolled in this study. After Leksell stereotaxic instrument was installed, appropriate parameter scanning was performed and then Surgiplan Stereotactic planning system (referred to as SPS), intraoperative intracranial deep microelectrode recording electrophysiological validation, the thalamus Vim and Vo nuclear RF rupture, postoperative MR review to verify the accuracy of the target. Results MRI scanning of 33 cases showed that the adjacent structures of AC, PC, basal ganglia and each gray matter mass were clearly displayed. The MR images were successfully imported into the SPS surgical planning system. Conclusion Using the Pd + T2_tse sequence, the magnetic resonance imaging system can locate the target more quickly and accurately.
