目的探讨血清乳酸脱氢酶水平在地中海贫血患儿临床诊断中的应用价值。方法选取广西壮族自治区桂林医学院附属医院儿科2013年8月—2016年8月接收的40例地中海贫血患者为研究对象,其中非感染患儿30例为A组,合并感染患儿10例为B组;并抽取同期就诊的急性溶血患者(11例)与非溶血患者(39例)对照研究,分别作为C组与D组。对四组研究对象血清乳酸脱氢酶水平进行检测对比;并检测地中海贫血患儿贫血程度,观察其与血清乳酸脱氢酶水平相关性。结果 A组与B组的血清乳酸脱氢酶水平均明显高于D组(t=11.49,25.74,P<0.05);B组高于A组(t=18.62,P<0.05);B组与C组比较,差异无统计学意义(t=1.79,P>0.05)。轻、中度地中海贫血患儿血清乳酸脱氢酶水平(351.19±145.82)U/L,明显低于重度贫血患儿(450.81±204.13)U/L(t=8.13,P<0.05)。结论地中海贫血患者在血清乳酸脱氢酶水平上明显升高,且合并感染时升高更为明显,与贫血严重程度呈正相关,在地中海贫血患儿临床病情诊断中有重要观察价值。
Objective To investigate the clinical value of serum lactate dehydrogenase in the diagnosis of children with thalassemia. Methods Forty patients with thalassemia from August 2013 to August 2016 in the pediatric department of Affiliated Hospital of Guilin Medical College of Guangxi Zhuang Autonomous Region were enrolled in this study. A group of 30 non-infected children was Group A, and 10 children with combined infection were B The patients in acute hemolysis (n = 11) and non-hemolysis (n = 39) were selected as C and D groups respectively. The levels of serum lactate dehydrogenase in four groups of subjects were tested and compared. The level of anemia in children with thalassemia was also detected, and the correlation with serum lactate dehydrogenase was observed. Results Serum lactate dehydrogenase levels in group A and group B were significantly higher than those in group D (t = 11.49, 25.74, P <0.05); group B was significantly higher than group A (t = 18.62, P <0.05) There was no significant difference between group C and group C (t = 1.79, P> 0.05). Serum LDH level in children with mild or moderate thalassemia was (351.19 ± 145.82) U / L, which was significantly lower than that in children with severe anemia (450.81 ± 204.13) U / L (t = 8.13, P <0.05). Conclusion The level of serum LDH in patients with thalassemia is significantly higher than that in patients with IDA, and the level of serum LDH is higher in co-infected patients. It is positively correlated with the severity of anemia and has important value in the diagnosis of children with thalassemia.
