目的采用12导联动态心电图(12-Holter)对心血管病患者和非心血管病患者进行房颤(AF)及阵发性房颤(PAF)检测分析,评价12-Holter对AF及PAF的诊断价值。方法对2275例心内科住院及门诊患者进行12-HOLTER检测,诊断AF及PAF并对“房颤负荷”指标作定量分析。结果①12-Holter共检出AF364例,占总病例数16.00%;其中PAF 84例,占总病例数3.69%及AF病例数的23.08%;②12-HOLTER对AF的检出率为16.00%,对PAF的检出率为3.69%,均明显高于12-ECG及心电监护的检出率(12.92%,12.79%及3.02%3,.85%,p<0.01);③AF及PAF多发生于心血管病患者,其中高血压患者较多(AF153例,42.03%;PAF36例,42.86%);年龄>70岁患者的AF及PAF发生率较高(AF 207例,25.06%;PAF54例,26.09%);女性AF及PAF患者的发生率略高于男性(女性AF213例,17.47%;PAF54例4,.43%;男性AF151例,14.30%;PAF30例,2.84%p;<0.01);④发生PAF的患者均具有一定强度的“房颤负荷”。结论 12-HOLTER可发现并明确诊断AF及PAF并可进行“房颤负荷”指标的定量检测分析。
Objective To evaluate the effects of 12-Holter on AF and PAF in patients with cardiovascular diseases and non-cardiovascular patients by 12-Holter electrocardiogram (AF) and paroxysmal atrial fibrillation (PAF) Diagnostic value. Methods A total of 2275 inpatients and outpatients from Department of Cardiology were enrolled in the 12-HOLTER test. AF and PAF were diagnosed and the “Atrial Fibrillation Load” index was quantitatively analyzed. Results ①12 AF-Holter detected AF364 cases, accounting for 16.00% of the total cases; PAF 84 cases, accounting for 3.69% of the total number of cases and 23.08% of the number of AF cases; ② 12-HOLTER detection rate of AF was 16.00% The detection rate of PAF was 3.69%, which were significantly higher than those of 12-ECG and ECG (12.92%, 12.79%, 3.02%, 3.85%, p <0.01) The incidence of AF and PAF was higher in patients with cardiovascular disease (AF153, 42.03%; PAF, 36.82%, 42.86%); AF and PAF were higher in patients older than 70 years (AF 207, 25.06%; PAF54, 26.09 %). The incidence of AF in women and patients with PAF was slightly higher than that in men (AF213 cases, 17.47%; PAF54 cases, 4.43%; male AF151 cases, 14.30%; PAF30 cases, 2.84% p; PAF patients have a certain intensity of “AF load ”. Conclusions 12-HOLTER can detect and diagnose AF and PAF clearly, and can be used to quantitatively analyze the “Atrial Fibrillation Load” index.
