目的探讨深度水解蛋白配方在早产儿早期喂养中的作用。方法对2010年6月至2011年5月本院儿科新生儿病房住院的早产儿进行研究,将胎龄<34周早产儿及晚期早产儿(胎龄34~36周)分别随机分为深度水解蛋白配方喂养组(观察组)及早产儿配方喂养组(对照组),监测生长发育指标和生后2天内、7~10天、12~15天血清胃泌素和胃动素水平。结果 (1)胎龄<34周早产儿观察组胎便排净时间和达到全肠道喂养时间均较对照组缩短[(4.6±1.5)天比(6.1±2.2)天,(13.6±2.8)天比(16.1±4.0)天],喂养不耐受发生率较对照组低(55.6%比84.6%),自主排便次数较对照组多[(1.8±1.0)次/天比(1.2±0.6)次/天],生后2天内及7~10天胃动素水平(pg/ml)高于对照组[(348.7±177.7)比(245.4±96.6),(493.3±235.9)比(343.2±141.6)],12~15天胃泌素(pg/ml)水平高于对照组[(75.1±21.8)比(59.0±16.6)],差异均有统计学意义,P均<0.05。(2)晚期早产儿观察组和对照组之间各项观察指标差异均无统计学意义,P均>0.05。结论与早产儿配方相比,早期给予深度水解蛋白配方喂养对于胎龄较小的早产儿可降低喂养不耐受的发生率,缩短达到全肠道喂养的时间。
Objective To investigate the effect of deeply hydrolyzed protein formula on early feeding of premature infants. Methods From June 2010 to May 2011, we investigated the hospitalization of premature infants in pediatric neonatal wards, and randomly divided into three groups: premature infants of gestational age <34 weeks and preterm infants of late gestational age (gestational age of 34 to 36 weeks) (Control group) and the formula feeding group (control group). The indexes of growth and development and the level of serum gastrin and motilin within 7 days, 12 days and 12-15 days were monitored within 2 days after birth. Results (1) Shortened urination time and total intestine feeding time in preterm infants of gestational age <34 weeks were shorter than those in control group [(4.6 ± 1.5) days vs (6.1 ± 2.2 days), (13.6 ± 2.8 days (16.1 ± 4.0) days]. The incidence of feeding intolerance was lower than that of the control group (55.6% vs 84.6%) and the number of spontaneous defecation was (1.8 ± 1.0) times / day (1.2 ± 0.6) times more than that of the control group / Day], the level of motilin (pg / ml) within 2 days and 7-10 days after birth was higher than that of the control group [(348.7 ± 177.7) vs (245.4 ± 96.6), (493.3 ± 235.9) vs (343.2 ± 141.6) ], And the levels of gastrin (pg / ml) in 12-15 days were significantly higher than those in the control group [(75.1 ± 21.8) vs (59.0 ± 16.6)], all of which were statistically significant (P <0.05). (2) There was no significant difference between the observation group and the control group in the late preterm children (P> 0.05). Conclusion Compared with the formula of preterm infant, the early formulation of deep hydrolyzed protein formula can reduce the incidence of feeding intolerance and shorten the time of full gut feeding in premature infants of smaller gestational age.
