目的探讨膨肺吸痰法在机械通气治疗慢性阻塞性肺疾病患者的疗效。方法方便选取2016年1—4月该院呼吸ICU收住院的48例行机械通气治疗的慢性阻塞性肺疾病患者,随机分成两组,研究组(n=24例)采用膨肺吸痰法,对照组(n=24例)采用常规吸痰法。观察两组吸痰前后血气分析(SPO2、Pa O2、Pa CO2)以及肺部感染发生率、肺不张发生率指标变化。结果两组患者吸痰后SPO2、Pa O2指标均较吸痰前升高,而Pa CO2较吸痰前下降(P<0.05),研究组较对照组升高或下降的更为明显(P<0.05),研究组肺部感染发生率和肺不张发生率分别是16.67%和12.50%,对照组肺部感染发生率和肺不张发生率分别是45.83%和37.50%,研究组较对照组肺部感染、肺不张的发生率低(P<0.05)。结论膨肺吸痰能有效改善患者血气分析指标,改善通气和换气,预防低氧血症,降低肺不张和肺部感染的发生率,膨肺吸痰法较普通吸痰法有较好的疗效。
Objective To investigate the curative effect of expanding suction suction sputum method in patients with chronic obstructive pulmonary disease treated by mechanical ventilation. Methods Convenient Select 48 patients with chronic obstructive pulmonary disease undergoing mechanical ventilation in hospital from January to April 2016 were randomly divided into two groups. The study group (n = 24) Control group (n = 24 cases) using conventional suction method. The blood gas analysis (SPO2, Pa O2, Pa CO2) before and after sputum aspiration and the incidence of pulmonary infection and the incidence of atelectasis were observed. Results The SPO2 and Pa O2 levels in both groups were significantly higher than those before sputum aspiration and Pa CO2 decreased compared with before sputum aspiration (P <0.05) 0.05). The incidence of pulmonary infection and atelectasis in study group were 16.67% and 12.50% respectively. The incidence of pulmonary infection and the incidence of atelectasis in control group were 45.83% and 37.50% respectively. Compared with control group Pulmonary infection, the incidence of atelectasis was low (P <0.05). Conclusion Expansion of suction sputum can effectively improve blood gas analysis in patients with indicators to improve ventilation and ventilation to prevent hypoxemia and reduce the incidence of pulmonary atelectasis and pulmonary infection, lung suction sputum method is better than ordinary suction method Efficacy.
