目的:研究胰岛素抵抗与脂代谢异常在妊娠期高血压疾病(HDCP)发病中的作用及相关性。方法:以62例HDCP患者为研究对象,并以同期正常孕妇32例为对照组。检测两组空腹血糖(FBG)、胰岛素(FINS)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)、载脂蛋白A-1(apoA-1)、载脂蛋白B(apoB)的含量,以HOMA指数为胰岛素抵抗指标,分析以上各指标在两组孕妇中的差异及其相关性。结果:HDCP组与正常妊娠组相比,其HOMA指数、TG、LDL、apoB显著升高,且随病情的加重逐渐升高;HDL、apoA-1显著降低,且随病情加重而逐渐降低。HOMA指数与脂代谢各指标间具有显著相关性。结论:HDCP患者存在胰岛素抵抗与脂代谢异常,并且两者具有相关性。
Objective: To study the role and correlation of insulin resistance and lipid metabolism in the pathogenesis of HDCP. Methods: Sixty-two patients with HDCP were studied, and 32 normal pregnant women in the same period were taken as control group. The levels of fasting blood glucose (FBG), insulin (FINS), total cholesterol (TC), triglyceride (TG), high density lipoprotein (HDL), low density lipoprotein (LDL), apolipoprotein A-1 apoA-1), apolipoprotein B (apoB) levels, HOMA index as an indicator of insulin resistance, the above indicators in the two groups of pregnant women, the differences and their correlation. Results: The HOMA index, TG, LDL and apoB of HDCP group were significantly higher than those of normal pregnancy group, and gradually increased with the severity of HDCP. HDL and apoA-1 were significantly decreased and gradually decreased with the progression of HDCP. HOMA index and lipid metabolism between the indicators have a significant correlation. Conclusion: There is a correlation between insulin resistance and lipid metabolism in patients with HDCP.
