目的探讨24小时动态脑电图(AEEG)监测对小儿癫痫诊断及疗效评估的价值。方法对86例拟诊为癫痫(EP)和50例EP治疗后复查的病例均进行24小时动态脑电图监测,并与常规脑电图(REEG)检查结果对比分析。结果常规脑电图检查未发现EP放电波的86例疑诊病例中,监测到癫痫波65例,非痫样放电波5例,总阳性率为81.39%,其中13例同时扑捉到临床发作。并对其中50例EP治疗2年后复查的病例先予常规脑电图检查,对常规脑电图检查未发现EP放电波者复查动态脑电图,异常32例,阳性率为64%。与常规脑电图检查结果比较有显著性差异。结论 24小时动态脑电图(AEEG)监测可提高小儿EP诊断阳性率和准确率,对小儿癫痫诊断和分型及临床疗效评估具有重要价值。
Objective To investigate the value of 24-hour dynamic electroencephalogram (EEG) monitoring in the diagnosis and evaluation of pediatric epilepsy. Methods Twenty-six cases of epilepsy (EP) and 50 cases of EP after treatment were monitored by dynamic EEG for 24 hours, and compared with conventional electroencephalogram (REEG). Results Of the 86 suspected EP cases, 65 cases of epileptic waves and 5 cases of non-epileptiform discharge were detected in conventional electrocardiogram (EEG). The total positive rate was 81.39%. Thirteen of them also caught clinical seizures . 50 cases of EP after 2 years of review of the cases prior to conventional electroencephalography, conventional electroencephalography did not find EP wave discharge were reviewed dynamic EEG, abnormalities in 32 cases, the positive rate was 64%. Compared with the results of conventional electroencephalography, there are significant differences. Conclusion The 24-hour dynamic electroencephalogram (AEEG) monitoring can improve the positive rate and accuracy of EP diagnosis in children, which is of great value in the diagnosis and classification of pediatric epilepsy and clinical efficacy evaluation.
