目的:探讨妊娠期糖尿病(GDM)患者腹部皮下和网膜脂肪组织Omentin基因表达的差异及其与糖代谢水平的相关性。方法:将43例GDM孕妇与43例正常孕妇(NGT)按年龄、产次、分娩前BMI、分娩孕周配对后,测量两组产前空腹血脂水平,并于剖宫产术中留取研究对象腹部皮下和网膜脂肪组织,应用实时定量-聚合酶链反应检测不同部位脂肪组织OmentinmRNA表达水平,分析Omentin mRNA表达水平与OGTT结果和产前脂代谢的相关性。结果:①在GDM人群中,皮下及网膜脂肪组织的Omentin mRNA的表达水平均显著低于NGT组对应部位的表达水平。②在单因素分析中,网膜脂肪组织的Omentin mR-NA表达量与OGTT空腹血糖和餐后2 h血糖呈显著负相关;而腹部皮下脂肪组织的Omentin mRNA表达量则与OGTT餐后2 h血糖水平以及产前HDL-C水平呈显著相关性。③进一步进行多元逐步回归分析显示,网膜脂肪组织和皮下脂肪组织Omentin mR-NA表达量均是OGTT餐后2 h血糖水平独立预测因子,网膜脂肪组织Omentin mRNA表达量对OGTT餐后2 h血糖水平影响更大。结论:网膜脂肪组织Omentin mRNA表达与糖代谢的关系可能比皮下脂肪组织更为密切。
Objective: To investigate the difference of Omentin gene expression in subcutaneous subcutaneous and omental adipose tissue in patients with gestational diabetes mellitus (GDM) and its correlation with glucose metabolism. Methods: 43 cases of GDM pregnant women and 43 normal pregnant women (NGT) were divided into two groups according to their age, parity, BMI before birth and gestational weeks of delivery. The prenatal fasting blood lipid levels were measured in both groups and were collected during cesarean section Objective To detect the expression of Omentin mRNA in adipose tissue of subcutaneous and omental adipose tissue in the abdomen, detect the expression of Omentin mRNA in different tissues by real-time quantitative polymerase chain reaction and analyze the correlation between Omentin mRNA expression and prenatal lipid metabolism. Results: ①In GDM group, the expression levels of Omentin mRNA in subcutaneous and omental adipose tissue were significantly lower than those in NGT group. ② In univariate analysis, Omentin mR-NA expression in omental adipose tissue was negatively correlated with fasting blood glucose (OGTT) and postprandial blood glucose (2h), while Omentin mRNA expression in abdominal subcutaneous adipose tissue was negatively correlated with OGTT Blood glucose levels and prenatal HDL-C levels were significantly correlated. ③ Further multivariate stepwise regression analysis showed that Omentin mR-NA expression in both omental adipose tissue and subcutaneous adipose tissue were both independent predictors of OGTT 2-hour postprandial blood glucose level. Omentin mRNA expression in omental adipose tissue was significantly decreased after 2 hours of OGTT Blood glucose levels have a greater impact. Conclusion: The relationship between omentin mRNA expression and glucose metabolism may be more closely related to subcutaneous adipose tissue in omental adipose tissue.
