目的:探讨痰热清注射液联合高压氧(HBO)治疗对肺癌患者放射性肺炎及放射性肺纤维化的影响及其作用机制。方法:选取2017年1月至2019年1月于蓬莱市人民医院肿瘤科确诊为肺癌且接受三维适型放疗的患者86例。采用随机数字表法分为观察组和对照组,每组43例。2组患者均接受常规对症治疗和放射治疗,对照组从放疗开始后给予痰热清注射液治疗,观察组在对照组的治疗基础上加用HBO治疗。统计2组患者治疗后放射性肺炎发生率及放射性肺纤维化发生时间;于放疗结束后2、4、6周分别抽取空腹静脉血5 ml,采用酶联免疫吸附(ELISA)法测定血清中转化生长因子-β(TGF-β)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)以及血浆血管紧张素转换酶(ACE)水平。结果:治疗2~6个疗程后,观察组患者治疗后放射性肺炎、放射性肺纤维化发生率明显低于对照组,平均发生时间晚于对照组,差异均有统计学意义(n P<0.05)。观察组患者治疗2、4、6周后血清ACE、TGF-β和TNF-α水平明显高于对照组,IL-6水平明显低于对照组,差异均有统计学意义(n P<0.05或n P<0.01)。n 结论:痰热清注射液联合HBO治疗肺癌患者放射性肺损伤,能明显降低放射性肺炎发生率,并延缓放射性肺纤维化的发生,对临床具有指导意义。“,”Objective:To investigate the effects of Tanreqing injection combined with hyperbaric oxygen (HBO) on radiation pneumonitis and radiation pulmonary fibrosis in patients with lung cancer.Methods:A total of 86 patients with non-small cell lung cancer admitted to the Department of Oncology of Penglai People’s Hospital from January 2017 to January 2019 were selected for three-dimensional conformal radiotherapy, who were divided into observation group and control group according to random number table method, with 43 cases each. The two groups were treated with conventional symptomatic therapy and radiotherapy. The control group was treated with Tanreqing injection after radiotherapy, and the observation group was treated with HBO on the basis of that of the control group. The incidence of radiation pneumonia and the occurrence time of radiation pulmonary fibrosis in the two groups were recorded. The fasting venous blood was collected 5 ml each time at 2, 4, and 6 weeks, respectively, after radiotherapy. The levels of transforming growth factor-β (TGF-β), interleukin-6 (IL-6), and tumor necrosis factor-α (TNF-α) in serum and angiotensin-converting enzyme (ACE) in plasma were measured by enzyme-linked immunosorbent assay (ELISA).Results:After 2 to 6 courses of treatment, the incidences of radiation pneumonitis and radiation pulmonary fibrosis in the observation group were significantly lower than those in the control group, and the average occurrence time was later than that in the control group (n P<0.05). The levels of ACE, TGF-β, and TNF-α in serum in the observation group were significantly higher than that in the control group, and the level of IL-6 was significantly lower than those in the control group (n P<0.05 orn P<0.01), 2, 4, and 6 weeks after radiotherapy.n Conclusion:Tanreqing injection combined with HBO can significantly reduce the incidence of radiation pneumonitis and delay the occurrence of radiation pulmonary fibrosis, which has guiding significance to clinical practice.
