目的通过阴道镜图像分析及定位活检结果,评价电子阴道镜检查在宫颈病变中的价值及应用。方法2004年7月-2009年9月对宫颈病变行阴道镜检查2086例,分析评价阴道镜检查效能及宫颈病变的构成比。结果2086例阴道镜检查患者中,有544例行活检术,病理结果显示,子宫颈鳞癌16例;宫颈上皮内病变(CIN)98例,其中CINⅠ级63例,CINⅡ级21例,CINⅢ级14例;人乳头状瘤病毒(HPV)感染44例,急慢性宫颈炎386例。阴道镜阳性预测值69.60%,阴性预测值96.42%,完全符合率为77.57%。结论阴道镜检查能及时早期发现宫颈微小病灶,指导选择性定位活检,对早期宫颈癌及癌前病变诊断有重要价值。
Objective To evaluate the value and application of electronic colposcopy in cervical lesions by colposcopy image analysis and biopsy results. Methods From July 2004 to September 2009, 2086 cases of cervical lesions were examined by colposcopy. The colposcopy efficacy and the ratio of cervical lesions were analyzed and evaluated. Results Among the 2086 cases with colposcopy, 544 cases were biopsied. The pathological results showed that there were 16 cases of cervical squamous cell carcinoma and 98 cases of cervical intraepithelial lesions (CIN), of which 63 cases were CINⅠ, 21 cases were CINⅡ, 14 cases; human papilloma virus (HPV) infection in 44 cases, 386 cases of acute and chronic cervicitis. The colposcopy positive predictive value was 69.60%, the negative predictive value was 96.42%, and the complete coincidence rate was 77.57%. Conclusions Colposcopy can detect tiny lesions of the cervix in time and direct the selective biopsy, which is of great value in the diagnosis of early cervical cancer and precancerous lesions.
