目的 通过检测血浆高迁移率族蛋白B1(high mobility group protein B1,HMGB1)水平结合冠脉内超声技术,评价冠心病、不稳定性心绞痛合并睡眠呼吸暂停低通气综合征患者冠脉内斑块负荷情况.方法 选择冠脉造影单支病变,临床诊断为冠心病、不稳定性心绞痛患者85例,其中的39例合并睡眠呼吸暂停低通气综合征作为研究组,46例未合并睡眠呼吸暂停低通气综合征为对照组,分析两组患者HMGB1水平以及冠脉内超声评价斑块负荷情况.结果 研究组HMGB1水平明显高于对照组,冠脉内超声提示研究组管腔面积和冠状动脉斑块纤维帽厚度明显低于对照组,斑块负荷、重构指数和斑块面积明显大于对照组.结论 冠心病、不稳定性心绞痛患者及高水平HBGB1与合并睡眠呼吸暂停低通气综合征冠脉内斑块易损性临床相关.“,”To investigate the blood Inflammatory markers:plasma level of high mobility group protein B1 (HMGPB1) andserum C-reactivie protein (CRP)of the patients with unstable angina pectoris (UAP)combined with obstructive sleep apnea hypopnea syndrome (OSAHS)and analye the correlaton of HMGPB1 with the plaque formation indicis. Methods 85 patients UAP patients were divided into demographically matched 2 groups :OSAHS group(n=39) and non-OSAHS group (n=46). All patients underwent coronary angiography, and the plaque volume, plaque load, and reconstruction index were calculated. Peripheral fasting blood samples were collected to undergo examination of HMGPB1 by enzyme-linked immunosorbent assay and C-reactive protein (CRP) by using an enzymatic immunoassay turbidimetric method (reagent: Orion, Finnish; apparatus: Roche Cobas 6000, Penzberg, Germany). SPSS 22.0 was used to analyze the relevant correlation. Results The levels of HMGPB1 and CRP of the OSAHS group were both signficantly higher than those of the non-OSAHS group (both P<0.05).Pearson linear correlation analysis showed that the HMGB1 level of the OSAHS group was positively correlated with the reconstruction index and Eccentricity index (r=0.341,P=0.030; r=0.312, P=0.025), and negatively correlated with the Fiber cap thickness (r=-0.299,P=0.040). Conclusion group. Conclusion coronary heart disease, unstable angina pectoris and high levels of HBGB1 are clinically relevant to the plaque vulnerability in coronary artery disease with sleep apnea hypopnea syndrome.
