目的:探讨抗菌药物不同给药时间对手工刮治重度慢性牙周炎短期疗效及牙周膜细胞增生的影响。方法:选择在我院治疗的重度慢性牙周炎患者63例,按随机数字表法分为A组(31例)和B组(32例),A组患者在手工刮治的同时给予抗菌药物治疗,B组患者在手工刮治完成后给予抗菌药物治疗。对两组患者牙周检查情况、咀嚼功能、牙槽骨高度、牙周膜细胞增殖情况以及生存质量进行评估。结果:治疗后A组患者平均探诊深度、平均邻面探诊深度、探诊深度超过5 mm位点的百分比、邻面探诊深度>5 mm位点的百分比、平均出血指数、探诊出血位点百分比均显著低于B组(P<0.05),牙槽骨高度显著高于B组(P<0.05),咀嚼效率显著高于B组(P<0.05),生存质量各维度得分均显著高于B组(P<0.05),牙周膜细胞数量在第3、5天均显著高于B组(P<0.05)。结论:刮治同时应用全身抗菌药物有助于抑制牙槽骨高度降低,降低牙周袋深度,提高咀嚼效率,改善患者生存质量,为牙周膜细胞的增殖创造更加适宜的条件。
Objective: To investigate the effect of different administration time of antimicrobial agents on short-term curative effect of hand-scraping and treating chronic periodontitis and proliferation of periodontal ligament cells. Methods: Sixty-three patients with severe chronic periodontitis who were treated in our hospital were divided into group A (n = 31) and group B (n = 32) by random number table. Patients in group A were given anti-bacterial drugs Treatment, patients in group B were treated with antimicrobial therapy after the hand scraping was completed. Periodontal examination, masticatory function, alveolar bone height, periodontal ligament cell proliferation and quality of life were evaluated in both groups. Results: After treatment, the average exploration depth, the average depth of proximal probing, the percentage of probing depth more than 5 mm, the percentage of probing depth> 5 mm, the average bleeding index, the hemorrhage in exploration (P <0.05). The height of alveolar bone was significantly higher than that of group B (P <0.05), and the chewing efficiency was significantly higher than that of group B (P <0.05). The scores of quality of life were significantly different Higher than group B (P <0.05), the number of periodontal ligament cells was significantly higher than that of group B on the 3rd and 5th day (P <0.05). Conclusion: Scraping and applying systemic antimicrobial agents can help to reduce the alveolar bone height, reduce the depth of periodontal pocket, improve the masticatory efficiency, improve the quality of life of patients, and create more appropriate conditions for the proliferation of periodontal ligament cells.
