目的评估腹腔镜与开腹手术患者术前抑郁、焦虑的心理状况。方法 2007年10月-2008年9月将收治的60例行胆囊术患者随机分为腹腔镜组与开腹组,各30例,术前1d使用抑郁自评量表和焦虑自评量表对患者的焦虑、抑郁水平进行评估。结果两组患者抑郁、焦虑水平均高于全国平均水平(常模);开腹组术前抑郁、焦虑水平高于腹腔镜组但无统计学意义(P>0.05)。开腹组急诊手术患者抑郁水平高于择期手术(P<0.05),焦虑水平无明显差异。开腹组女性抑郁和焦虑水平均高于男性(P<0.05);腹腔镜组女性抑郁水平高于男性(P<0.05),焦虑水平男女之间差异无统计学意义。开腹组40.0%的患者处于抑郁状态,高于腹腔镜组的16.7%。结论医师术前应注意与患者积极沟通,消除患者对手术的顾虑,尤其是女性患者及急诊手术患者。
Objective To evaluate the psychological status of preoperative depression and anxiety in patients undergoing laparoscopy and laparotomy. Methods From October 2007 to September 2008, 60 patients undergoing gallbladder surgery were randomly divided into laparoscopic group and laparotomy group, 30 cases in each group. The self-rating depression scale and self-rating anxiety scale Patients with anxiety, depression levels were assessed. Results Both depression and anxiety levels were higher than the national average (norm) in both groups. Preoperative depression and anxiety in the open group were significantly higher than those in the laparoscopic group (P> 0.05). The level of depression in patients undergoing open surgery was higher than that on elective surgery (P <0.05), and there was no significant difference in anxiety. The levels of depression and anxiety in open group were higher than those in male (P <0.05). The level of depression in female group was higher than that in male group (P <0.05). There was no significant difference between men and women in anxiety level. 40.0% of the patients in the open group were in depression, which was higher than that of the laparoscopic group (16.7%). Conclusion The physician should pay attention to the positive communication with the patient before surgery to eliminate the patients’ concerns about the operation, especially for female patients and emergency operation patients.
