目的比较胰岛素垂直皮下注射法与斜刺皮下注射法对老年糖尿病的局部疼痛和皮下出血情况。方法回顾性分析2010年3月—2013年6月实施皮下注射胰岛素的70例老年糖尿病患者的临床资料,随机分为对照组与观察组各35例。依照患者的实际状况分别选择普通胰岛素、甘舒霖30R、甘舒霖R以及诺和灵30R实施皮下注射,注射位置选择三角肌下缘,选择Bd针头8 mm(31 g)。观察组采用垂直注射法进行胰岛素注射。对照组采用斜刺注射法进行胰岛素注射。持续治疗2个月后,对比两组患者注射位置的疼痛情况与皮下出血情况。计数资料采用χ2检验,P<0.05为差异有统计学意义。结果观察组重度局部疼痛的占8.57%,对照组占40.00%,两组比较差异有统计学意义(P<0.05)。观察组患者注射局部皮下出血的占8.57%,明显低于对照组的37.14%,两组比较差异有统计学意义(P<0.05)。结论采用胰岛素垂直皮下注射法能有效缓解老年糖尿病患者疼痛,同时能有效降低注射后皮下出血,从而有效提升患者治疗依从性,值得临床推广应用。
Objective To compare the local pain and subcutaneous bleeding in elderly patients with diabetes mellitus by subcutaneous injection of insulin and subcutaneous injection of diabetics. Methods The clinical data of 70 elderly diabetic patients who underwent hypodermic injection of insulin from March 2010 to June 2013 were retrospectively analyzed. The patients were randomly divided into control group and observation group of 35 cases each. According to the actual conditions of patients were selected ordinary insulin, Ganshu Lin 30R, Ganshu Lin R and Novolin 30R subcutaneous injection, injection position selected deltoid lower edge, select Bd needle 8 mm (31 g). Observation group using vertical injection insulin injection. Control group using oblique injection of insulin injection. After 2 months of continuous treatment, the pain and subcutaneous bleeding were compared between the two groups at the injection site. Count data using χ2 test, P <0.05 for the difference was statistically significant. Results The severe pain in the observation group accounted for 8.57%, while the control group accounted for 40.00%, the difference between the two groups was statistically significant (P <0.05). The proportion of subcutaneous hemorrhage in observation group was 8.57%, which was significantly lower than that in control group (37.14%). There was significant difference between the two groups (P <0.05). Conclusion The vertical subcutaneous injection of insulin can effectively relieve the pain of senile diabetic patients and can effectively reduce the subcutaneous hemorrhage after injection so as to effectively improve the treatment compliance of patients and is worthy of clinical application.
