目的探索原发性高血压患者在亚极量运动时血压变化幅度与踝臂动脉脉搏波传导速度(baPWV)的关系。方法连续入选2014年12月至2015年5月北京军区总医院心内科就诊的原发性高血压患者(原发性高血压组)58例和正常血压者(正常对照组)32人,年龄(53.3±7.7)岁。根据是否使用降压药将原发性高血压组分为新发组(未服用降压药25例)和治疗组(已服用药33例)。采用Pearson相关及多元回归分析,观察亚极量运动收缩压与baPWV的相关性。结果 Pearson相关性分析结果显示,原发性高血压患者亚极量运动收缩压(r=0.712)和静息收缩压(r=0.599)与baPWV相关(均P<0.05)。多元线性回归分析结果显示,在原发性高血压患者中,空腹血糖(β=0.410)和亚极量运动收缩压(β=0.639)与baPWV相关(均P<0.01)。结论原发性高血压患者亚极量运动收缩压与baPWV密切相关。
Objective To explore the relationship between the amplitude of blood pressure and the ankle brachial artery pulse wave velocity (baPWV) in patients with essential hypertension during submaximal exercise. Methods Fifty-eight patients with essential hypertension (essential hypertension group) and 32 patients with normal blood pressure (normal control group) were enrolled in the Department of Cardiology, Beijing Military Region General Hospital from December 2014 to May 2015. The patients with age ( 53.3 ± 7.7) years old. According to whether to use antihypertensive drugs, the patients with essential hypertension were divided into new-onset group (25 cases without antihypertensive drugs) and treatment group (33 cases had taken drugs). Pearson correlation and multiple regression analysis were used to observe the association between suboptimal exercise systolic blood pressure and baPWV. Results Pearson correlation analysis showed that suboptimal exercise systolic pressure (r = 0.712) and resting systolic pressure (r = 0.599) were correlated with baPWV in patients with essential hypertension (all P <0.05). Multiple linear regression analysis showed that fasting blood glucose (β = 0.410) and suboptimal systolic blood pressure (β = 0.639) were correlated with baPWV in patients with essential hypertension (all P <0.01). Conclusion The sub-maximal exercise systolic pressure in patients with essential hypertension is closely related to baPWV.
