目的探讨压力超负荷下血管紧张素受体1(AT1)拮抗剂缬沙坦对心钠素(ANF)、人血管紧张素Ⅱ(AngⅡ)水平的影响。方法 24只3月龄健康SD雄性大鼠,采用不完全结扎腹主动脉方法建立大鼠高血压心肌肥厚模型,将其随机分为手术组(不完全缩窄腹主动脉)、手术加药物组[缬沙坦1 mg/(kg·d)]及正常对照组,每组8只。鼠尾测压仪监测术前及术后3、7、14、28 d各组大鼠尾动脉血压;测定心肌肥厚指数;应用放射免疫分析法测定肥厚心肌ANF、AngⅡ水平。结果术后3、7、14、28 d正常对照组和手术加药物组收缩压(SBP)、舒张压(DBP)显著低于手术组,差异有统计学意义(P<0.01);术后28 d手术加药物组与正常对照组血压比较差异无统计学意义(P>0.05)。手术组心肌肥厚指数为(32.9±1.38)mg/g,心肌ANF含量为(347.12±23.25)pg/ml,心肌AngⅡ含量为(98.36±10.61)pg/ml,显著高于手术加药物组[(27.66±2.14)mg/g、(161.92±10.69)pg/ml、(48.37±9.41)pg/ml]和正常对照组[(25.20±1.93)mg/g、(93.58±2.49)pg/ml、(35.33±6.56)pg/ml],差异有统计学意义(P<0.05)。手术加药物组的心肌ANF和AngⅡ含量高于正常对照组,差异有统计学意义(P<0.05)。结论心肌肥厚指数及ANF、AngⅡ含量可作为间接判断高血压心肌肥厚程度的指标,缬沙坦能削弱心肌肥厚的形成,并能降低心脏局部ANF、AngⅡ水平。
Objective To investigate the effect of valsartan, an antagonist of angiotensin Ⅱ receptor 1 (AT1), on the levels of atrial natriuretic peptide (ANF) and human angiotensin Ⅱ (Ang Ⅱ) under pressure overload. Methods Twenty-four healthy male Sprague-Dawley rats of 3 months old were used to establish model of hypertensive cardiac hypertrophy in rats by incomplete ligation of the abdominal aorta. They were randomly divided into operation group (incomplete abdominal constriction), operation plus drug group [Valsartan 1 mg / (kg · d)] and normal control group, 8 in each group. The rat caudal pressure manometer was used to monitor the tail arterial pressure before and 3, 7, 14 and 28 days after operation. Cardiac hypertrophy index was measured. The levels of ANF and AngⅡ in hypertrophic myocardium were measured by radioimmunoassay. Results The systolic blood pressure (SBP) and diastolic blood pressure (DBP) of the normal control group and the operation plus drug group were significantly lower than those of the operation group at 3, 7, 14 and 28 days after operation (P <0.01) There was no significant difference in the blood pressure between the operation plus drug group and the normal control group (P> 0.05). The myocardial hypertrophy index in the operation group was (32.9 ± 1.38) mg / g, the myocardial ANF content was (347.12 ± 23.25) pg / ml and the myocardial AngⅡ content was (98.36 ± 10.61) pg / 27.66 ± 2.14 mg / g, 161.92 ± 10.69 pg / ml and 48.37 ± 9.41 pg / ml, respectively, compared with the control group [(25.20 ± 1.93) mg / g and 35.33 ± 6.56) pg / ml], the difference was statistically significant (P <0.05). The myocardial ANF and AngⅡ levels in the operation plus drug group were significantly higher than those in the normal control group (P <0.05). Conclusions Myocardial hypertrophy index, ANF and Ang Ⅱ levels can be used as indexes to indirectly determine the degree of myocardial hypertrophy in hypertensive patients. Valsartan can reduce the formation of cardiac hypertrophy and decrease the levels of ANF and AngⅡ in the heart.
