目的 探讨端粒酶活性在食管粘膜上皮不典型增生、食管鳞癌中的变化 ,同时研究端粒酶活性与癌的分化、浸润、转移的关系。方法 应用显微切割 TRAP 银染法对 4 5例食管鳞癌组织、癌旁非典型增生上皮及切缘的正常粘膜上皮进行定点端粒酶活性的检测。结果 食管正常粘膜上皮的端粒酶活性阳性率为 5 % (2 / 4 0 ) ,非典型增生上皮为 79 3% (2 3/ 2 9) ,癌组织为 82 2 % (37/ 4 5 )。非典型增生上皮的端粒酶活性明显高于正常上皮 ,差异有显著意义 (P <0 0 1)。同一癌组织中不同分化程度、不同浸润深度癌巢的端粒酶活性差异无显著意义 (P >0 0 5 )。有淋巴结转移者癌组织端粒酶活性明显高于无淋巴结转移者 ,差异有显著意义 (P <0 0 5 )。结论 食管鳞癌及癌旁不典型增生组织端粒酶活性明显增高。癌组织中端粒酶活性与分化程度无关 ,而与淋巴结转移有关
Objective To investigate the changes of telomerase activity in esophageal epithelial dysplasia and esophageal squamous cell carcinoma and to study the relationship between telomerase activity and differentiation, invasion and metastasis of esophageal mucosa. Methods Microdissection TRAP silver staining was used to detect the telomerase activity in 45 cases of esophageal squamous cell carcinoma, adjacent atypical hyperplastic epithelium and normal mucosal epithelium of the margin. Results The positive rate of telomerase activity in normal esophageal epithelium was 5% (2/40), that of atypical hyperplastic epithelium was 79 3% (2 3/29) and that of cancerous tissue was 82 2% (37/45) . The telomerase activity of atypical hyperplastic epithelium was significantly higher than that of normal epithelium (P <0.01). There was no significant difference in telomerase activity in cancer nests with different degree of differentiation and depth of invasion in the same cancer (P> 0.05). The telomerase activity in cancerous tissues with lymph node metastasis was significantly higher than those without lymph node metastasis, the difference was significant (P <0 05). Conclusion The telomerase activity in esophageal squamous cell carcinoma and paraneoplastic atypical hyperplasia was significantly higher. Telomerase activity in cancer tissue was not related to the degree of differentiation, but was related to lymph node metastasis
