目的探讨腰大池置管持续引流对动脉瘤性蛛网膜下腔出血并发脑积水的防治效果。方法选取2014年3月至2016年2月于盘锦市中心医院进行诊治的动脉瘤性蛛网膜下腔出血患者68例,随机分为试验组和对照组,每组34例。对照组进行常规治疗,试验组则在常规治疗基础上行腰大池置管持续引流术,比较两组患者头痛程度、大脑中动脉血流速度、脑血管痉挛及脑积水发生情况。结果试验组患者疼痛视觉模拟量表(VAS)评分明显低于对照组,大脑中动脉血流速度显著慢于对照组,差异均有统计学意义(均P<0.05);试验组患者脑血管痉挛发生率及慢性脑积水发生率均明显低于对照组,差异均有统计学意义(均P<0.05)。结论腰大池置管持续引流应用于动脉瘤性蛛网膜下腔出血患者中,能够减轻患者的头痛程度,改善大脑中动脉血流速度,降低脑血管痉挛及慢性脑积水发生率。
Objective To investigate the prevention and treatment of continuous drainage of lumbar cistern for aneurysmal subarachnoid hemorrhage complicated with hydrocephalus. Methods Sixty-eight patients with aneurysmal subarachnoid hemorrhage diagnosed and treated in Panjin Central Hospital from March 2014 to February 2016 were randomly divided into experimental group and control group with 34 cases in each group. The control group was treated routinely. In the experimental group, continuous ligation of the lumbar cistern was performed on the basis of routine treatment. The degree of headache, blood flow velocity of the middle cerebral artery, cerebral vasospasm and hydrocephalus were compared between the two groups. Results The pain visual analog scale (VAS) score of the experimental group was significantly lower than that of the control group, and the middle cerebral artery blood flow velocity was significantly slower than that of the control group (all P <0.05). In the experimental group, the cerebral vasospasm Incidence and chronic hydrocephalus incidence were significantly lower than the control group, the difference was statistically significant (P <0.05). Conclusion Continuous drainage of the lumbar cistern for patients with aneurysmal subarachnoid hemorrhage can reduce headache, improve blood flow velocity of the middle cerebral artery and reduce the incidence of cerebral vasospasm and chronic hydrocephalus.
