目的利用神经电生理检测技术评价硫辛酸治疗糖尿病周围神经病变患者的临床疗效。方法选择2014年2月—2015年2月菏泽市第三人民医院神经内科诊断糖尿病周围神经病变患者110例,按照入院先后顺序,依据随机数字表法,将患者分为观察组与对照组各55例。利用神经电生理检测技术测定正中神经、尺神经、胫神经、腓总神经的运动神经传导速度(MCV)与感觉神经的传导速度(SCV)。比较二组患者临床疗效差异。结果治疗前,二组患者年龄、性别、病程、正中神经、尺神经、胫神经、腓总神经的运动神经传导速度(MCV)与感觉神经的传导速度(SCV)差异无统计学意义(P>0.05),治疗后,观察组与对照组比较,患者正中神经、尺神经、胫神经、腓总神经的运动神经传导速度(MCV)与感觉神经的传导速度(SCV)明显减少,二组比较差异有统计学意义(P<0.05),观察组与对照组患者比较,主诉肢体麻木者7例(12.73%)比16例(29.10%),t=2.984;疼痛1例(1.82%)比5例(9.09%),t=3.051;烧灼感6例(10.91%)比14例(25.45%),t=2.995;肢体无力感者9例(16.36%)比18例(32.73%),t=3.052;均明显减少,二组患者治疗后差异有统计学意义(P<0.05)。结论硫辛酸注射液治疗糖尿病周围神经病变可以明显促进周围神经功能传导,起到加强改善糖尿病周围神经异常症状的临床作用。
Objective To evaluate the clinical efficacy of lipoic acid in the treatment of diabetic peripheral neuropathy by electrophysiology. Methods From Feb 2014 to Feb 2015, 110 patients with diabetic peripheral neuropathy diagnosed by Department of Neurology, Third People’s Hospital of Heze City were divided into observation group and control group according to the order of admission and random number table example. Motor nerve conduction velocity (MCV) and sensory nerve conduction velocity (SCV) of the median nerve, ulnar nerve, tibial nerve and common peroneal nerve were measured by electrophysiological detection. The difference of clinical efficacy between the two groups was compared. Results Before treatment, there was no significant difference in motor nerve conduction velocity (MCV) and sensory nerve conduction velocity (SCV) between the two groups in terms of age, sex, duration of disease, median nerve, ulnar nerve, tibial nerve and common peroneal nerve (P> 0.05). After treatment, the motor nerve conduction velocity (MCV) and sensory nerve conduction velocity (SCV) of the median nerve, ulnar nerve, tibial nerve and common peroneal nerve of the observation group and the control group were significantly decreased compared with the control group (12.73%) than 16 patients (29.10%), t = 2.984; pain in 1 case (1.82%) than in 5 cases (P <0.05), the observation group and control group patients, (9.09%), t = 3.051. There were 6 cases (10.91%) of burning sensation than 14 cases (25.45%), t = 2.995, 9 cases (16.36% ; Both were significantly reduced, the difference between the two groups after treatment was statistically significant (P <0.05). Conclusion Lipoic acid injection in the treatment of diabetic peripheral neuropathy can significantly promote peripheral nerve conduction, play a role in improving the clinical symptoms of diabetic peripheral nerve abnormalities.
