目的探讨超声与MRI检查在腹外型韧带样纤维瘤诊断中的应用价值。方法 78例腹外型韧带样纤维瘤患者,术前行超声检查36例,行MRI常规和增强扫描42例,回顾性分析超声和MRI影像图特点。结果 78例腹外型韧带样纤维瘤均为单发,超声诊断36例中,31例病灶内部回声不均匀,23例病灶内可见明显血流信号;MRI诊断42例,其病灶STIR以高信号为主,可见点片状、索条状低信号,42例均呈中等及以上强化;36例术前超声测量肿瘤直径[(4.38±1.81)cm]小于术后组织病理学检查实测结果[(6.25±1.26)cm](P<0.05),42例术前MRI扫描测量肿瘤直径[(6.11±1.02)cm]与术后组织病理学检查实测结果[(6.18±0.99)cm]比较差异无统计学意义(P>0.05)。结论超声对韧带样纤维瘤纤维成分显示良好,MRI诊断韧带样纤维瘤具有一定优势,可为术前评估及术后评价提供客观依据。
Objective To investigate the value of ultrasound and MRI in the diagnosis of extra-abdominal ligament-like fibroids. Methods Totally 78 patients with extra-abdominal ligament-like fibroids were examined with ultrasound and MRI before operation. Twenty-six patients underwent routine MRI and contrast-enhanced MRI. The features of ultrasound and MRI were retrospectively analyzed. Results 78 cases of extra-abdominal ligament-like fibroids were single, 36 cases of ultrasound diagnosis, 31 cases of internal echo unevenness, 23 cases showed significant blood flow signal; MRI diagnosis of 42 cases, the lesion STIR high signal (42 cases) were moderate and above; 36 cases of preoperative ultrasound measurement of tumor diameter (4.38 ± 1.81 cm) was less than the postoperative histopathological examination results [( 6.25 ± 1.26 cm] (P <0.05). There were no significant differences in tumor diameter measured by preoperative MRI ([6.11 ± 1.02] cm] and postoperative histopathological examination [(6.18 ± 0.99) cm] Significance (P> 0.05). Conclusion Ultrasound has a good effect on the fiber components of ligament-like fibroids. MRI can diagnose ligament-like fibroids with certain advantages. It can provide an objective basis for preoperative evaluation and postoperative evaluation.
