目的:探讨64层CT小肠成像对小肠梗阻的诊断价值。方法:回顾性分析经手术病理和临床随访证实33例小肠梗阻的多层CT小肠成像的图像。运用图像后处理技术,显示小肠梗阻的部位和原因。结果:33例肠梗阻中肠粘连11例,小肠肿瘤10例,腹外疝3例,肠系膜扭转3例,肠套叠2例,阑尾炎1例,克罗恩病1例,肠系膜血管栓塞并发麻痹性肠梗阻1例,粪石性肠梗阻1例。其中21例手术,对照手术结果定位诊断的准确率为100%(22/22),病因诊断的准确率为90.9%(20/22)。结论:64层CT小肠成像是一项无创、安全的新技术,对于明确小肠梗阻的部位、原因、程度以及梗阻段肠管的血供状况有较高的临床价值。
Objective: To investigate the diagnostic value of 64-slice CT small bowel imaging in small bowel obstruction. Methods: A retrospective analysis of 33 cases of small bowel obstruction confirmed by surgery, pathology and clinical follow-up images of multi-slice CT small bowel imaging. Use image post-processing techniques to show the site and cause of small bowel obstruction. Results: 33 cases of ileus, 11 cases of intestinal adhesion, 10 cases of intestinal tumor, 3 cases of abdominal hernia, 3 cases of mesenteric reversion, 2 cases of intussusception, 1 case of appendicitis, 1 case of Crohn’s disease and mesenteric vessel embolism with paralysis 1 case of intestinal obstruction, 1 case of dung intestinal obstruction. Among them, the accuracy rate of positioning and diagnosis of the operation results was 100% (22/22) in 21 cases and 90.9% (20/22) in the etiological diagnosis. Conclusion: 64-slice CT small bowel imaging is a noninvasive and safe new technique, which has a high clinical value for clarifying the location, cause and degree of small bowel obstruction and the blood supply of the intestinal tract in the obstruction section.
