目的 探讨新生儿缺氧缺血性脑病(HIE)的超声诊断及分度标准,为规范HIE的诊断和治疗提供临床参考.方法 选取60例临床确诊为HIE的新生儿为观察组,其中轻度23例,中度24例,重度13例;40例健康新生儿为对照组,以前囟为透声窗观察大脑前动脉(ACA)、大脑中动脉(MCA)中央支的血流动力学变化,并动态随访观察预后.结果 生后24h,HIE组Vs、Vd均明显下降,RI异常升高,与对照组比较差异显著(P<0.05).48h后轻度HIE Vs、Vd及RI接近正常或已恢复正常(P>0.05).生后96h中、重度HIE患儿Vs、Vd及RI与对照组及轻度HIE比较差异显著(P<0.05).重度HIE患儿MCA中央支的Vs、Vd下降程度较中度显著(P<0.05),重度HIE患儿ACA的RI升高程度较中度显著(P<0.05).结论 彩色多普勒超声可动态观察HIE的变化过程,对HIE的临床诊断及评价预后有重要的价值.“,”Objective To explore the ultrasonic diagnosis and indexing standard of hypoxic ischemic encephalopathy (HIE) and to provide clinical reference for the diagnosis and treatment of HIE.Methods Select 60 neonates with diagnosis of HIE as the observation group, mild 23, moderate 24, severe 13, 40 healthy neonates as control group, the anterior fontanel as the acoustic window to observe the hemodynamic changes of anterior cerebral artery (ACA) and the central branches of middle cerebral artery (MCA), dynamic follow-up of prognosis were observed.Results Compared with the control group, HIE group Vs and Vd decreased significantly after birth 24 hours, and the RI increased significantly (P<0.05);The mild HIE Vs, Vd and RI either closed to normal or returned to normal 48 hours later (P>0.05).Compared with the control group and the mild HIE, there were significant differences between Vs, Vd and RI with severe HIE 96 hours later (P<0.05).The Vs and Vd of the central branches of MCA in severe HIE decreased higher than the mild HIE (P<0.05), and the RI of ACA in severe HIE increased higher than the mild HIE (P<0.05).Conclusions The color Doppler ultrasound can dynamically observe the change of HIE, which is of great value for the clinical diagnosis and the prognosis of HIE.
