目的:探讨24小时动态脑电图(24小时AEEG)在脑死亡判定中的临床应用价值。方法:回顾分析我院收治7例深昏迷患者24小时AEEG与TCD(经颅多普勒超声)、BAEP(脑干听觉诱发电位)、SLSEP(正中神经短潜伏期体感诱发电位)检测情况,并对资料进行分析整理。结果:24小时AEEG监测示为2vu以下脑电活动,均为广泛重度异常动态脑电图。结合TCD、BAEP、SLSEP检测结果,最终临床判定脑死亡,终止临床治疗。结论:24小时AEEG对脑功能的早期判定、预后及“脑死亡”判定具有重要参考价值,结合TCD、BAEP、SLSEP检测可以弥补临床判定技术上的不足,更准确地对脑死亡进行判定。
Objective: To investigate the clinical value of 24-hour dynamic electroencephalogram (24-hour AEEG) in judging brain death. Methods: The data of AEEG and TCD (transcranial Doppler ultrasound), BAEP (brainstem auditory evoked potentials) and SLSEP (median nerve short latency somatosensory evoked potentials) in 7 patients with deep coma admitted to our hospital were retrospectively analyzed. Data analysis and finishing. Results: 24-hour AEEG monitoring showed 2vu EEG activity, are extensive and severe abnormal dynamic EEG. Combined with TCD, BAEP, SLSEP test results, the final clinical judgment of brain death, termination of clinical treatment. Conclusion: 24-hour AEEG plays an important role in early judgment of brain function, prognosis and judgment of brain death. Combined with TCD, BAEP and SLSEP, it can make up for the technical deficiency of clinical judgment and judge the brain death more accurately.
