目的 探讨过熟期白内障囊外摘除和人工晶状体植入的手术方法及术中、术后并发症发生的原因和治疗措施。方法 对 2 0 2例 (2 0 2只眼 )过熟期白内障患者施行白内障囊外摘除和后房型人工晶状体植入术。结果 开罐式截囊 10 6只眼 ,信封式截囊 81只眼 ,连续环形撕囊 15只眼。 12 3只眼采用双手压迫式娩核 ,其余患眼采用晶状体套圈娩核。除 12只眼因术中玻璃体脱出未植入人工晶状体外 ,其余患者均植入后房型人工晶状体。 5 9.9%患者术后视力≥ 0 .5。信封式截囊及套圈娩核造成晶状体悬韧带断裂及玻璃体脱出等并发症更少。结论 对于皮质完全液化的过熟期白内障宜采用信封式截囊 ,伴晶状体悬韧带松驰的过熟期白内障套圈娩核较双手压迫式娩核有效及安全
Objective To explore the surgical methods of extracapsular extracapsular cataract extraction and intraocular lens implantation and the causes and treatment measures of intraoperative and postoperative complications. Methods Cataract extracapsular cataract extraction and posterior chamber intraocular lens implantation were performed in 220 patients (220 eyes) with cataract. Results There were 106 open capsulotomy, 81 closed envelopes and 15 consecutive circular capsulorhexis. 12 eyes with both hands with oppressive delivery of nuclear, the rest of the affected eye using lens ring nuclear delivery. All patients underwent implantation of posterior chamber intraocular lens (IOL) except intraocular vitreous prolapse in 12 eyes. 5 9.9% of patients with visual acuity ≥ 0.5. Envelope-type capsulotomy and ring nucleus delivery caused by the lens suspensory ligament rupture and vitreous prolapse and other complications less. Conclusion For cortical complete liquefaction of over-mature cataract should use envelope-type capsulotomy, lens suspensory ligament with over-mature cataract ring nuclear delivery than the two-hand oppression of the delivery of nuclear effective and safe
