[目的]探讨妊娠期肝内胆汁瘀积症(ICP)患者血脂水平的变化及熊去氧胆酸治疗ICP的效果。[方法]62例ICP患者随机数字表法分为治疗组(29例)和观察组(33例),观察组采用常规西药治疗,治疗组在观察组治疗基础上加用熊去氧胆酸,另入选40例正常孕妇作为对照组,观察各组孕妇肝功能、血脂变化。[结果]①ICP患者肝功能及血脂水平均高于正常孕妇(P<0.05);②多元线性回归分析结果显示,总胆固醇及三酰甘油水平均与总胆红素水平存在线性相关(P<0.05),而与天冬氨酸转氨酶、丙氨酸氨基转移酶及总胆汁酸无关(P>0.05);③与治疗前比较,治疗后治疗组肝功能、血脂均显著降低(P<0.05),且低于观察组(P<0.05);观察组肝功能指标显著降低,但血脂水平差异无统计学意义(P>0.05)。[结论]ICP患者血脂水平升高,且与总胆红素水平线性相关,可以作为评估ICP严重程度的指标。熊去氧胆酸能改善ICP患者肝功能,降低总胆红素、胆汁酸及血脂水平。
[Objective] To investigate the changes of blood lipid levels in pregnant women with intrahepatic cholestasis (ICP) and the effect of ursodeoxycholic acid on ICP. [Methods] Sixty-two patients with ICP were divided into treatment group (n = 29) and observation group (n = 33) by random number table. The observation group was treated with conventional western medicine. The treatment group was treated with ursodeoxycholic acid Another 40 cases of normal pregnant women were selected as the control group, observed in each group of pregnant women, liver function, blood lipid changes. [Results] (1) The liver function and serum lipids in patients with ICP were higher than those in normal pregnant women (P <0.05). The results of multivariate linear regression analysis showed that the levels of total cholesterol and triglyceride were linearly correlated with total bilirubin (P <0.05 ), But not with aspartate aminotransferase, alanine aminotransferase and total bile acid (P> 0.05); ③Compared with before treatment, the liver function and blood lipid of the treatment group were significantly decreased (P <0.05) (P <0.05). The indexes of liver function in the observation group were significantly lower than those in the observation group (P <0.05), but there was no significant difference in the level of serum lipids (P> 0.05). [Conclusion] Patients with ICP have elevated serum lipids and have a linear correlation with total bilirubin levels, which can be used as an index to evaluate the severity of ICP. Ursodeoxycholic acid can improve liver function in patients with ICP, reduce total bilirubin, bile acid and blood lipid levels.
