目的:探讨鼻腔鼻窦鳞状细胞癌中P16蛋白表达和HPV16感染情况与肿瘤临床特征、预后的关系以及二者的相关性。方法:收集经病理科诊断为鼻腔鼻窦鳞状细胞癌的组织标本55例,并以鼻内翻性乳头状瘤20例、鼻息肉组织20例作为对照组,应用免疫组织化学法检测P16蛋白表达和HPV16感染情况。结果:P16及HPV16在鼻腔鼻窦鳞状细胞癌组织及内翻性乳头状瘤组织中的阳性率均显著高于鼻息肉组,差异有统计学意义(P<0.05或<0.01)。P16表达与肿瘤病理分级、临床分期具有相关性(P<0.05),而与性别、年龄、吸烟、肿瘤原发部位无关(P>0.05);HPV16感染与肿瘤病理分级、临床分期、性别、年龄、吸烟及肿瘤原发部位无关(P>0.05)。在鼻腔鼻窦鳞状细胞癌组织中,P16表达与HPV16感染呈显著正相关(r=0.483,P<0.01);KaplanMeier生存分析显示,P16、HPV16阳性组的平均生存期和中位生存期均高于同指标阴性组,差异均有统计学意义(P<0.05)。结论:P16蛋白表达失控、HPV16感染与鼻腔鼻窦鳞状细胞癌的发生、进展密切相关,并且二者具有一定相关性,P16/HPV16阳性提示预后趋势更好。
Objective: To investigate the relationship between P16 protein expression and HPV16 infection in nasal sinus squamous cell carcinoma and its clinical characteristics, prognosis and their relationship. Methods: 55 cases of nasal sinus squamous cell carcinoma diagnosed by pathology were collected. Twenty cases of nasal inverted papilloma and 20 cases of nasal polyp were used as control group. The expression of P16 protein was detected by immunohistochemistry And HPV16 infection. Results: The positive rates of P16 and HPV16 in nasal sinus squamous cell carcinoma and inverted papilloma were significantly higher than those in nasal polyps, the difference was statistically significant (P <0.05 or <0.01). P16 expression was correlated with pathological grade and clinical stage (P <0.05), but not with gender, age, smoking, primary tumor site (P> 0.05). The positive rate of HPV16 was correlated with tumor grade, clinical stage, sex, , Smoking and primary tumor sites (P> 0.05). The positive expression of P16 was positively correlated with HPV16 infection in nasal sinus squamous cell carcinoma (r = 0.483, P <0.01). KaplanMeier survival analysis showed that the average survival and median survival of P16 and HPV16 positive groups were both high In the same index negative group, the differences were statistically significant (P <0.05). Conclusion: The expression of P16 protein is out of control. HPV16 infection is closely related to the occurrence and progression of nasal sinus squamous cell carcinoma. The positive correlation between P16 / HPV16 and P16 / HPV16 suggests that prognosis is better.
