目的探讨自上而下法(Top-Down法)评定测量不确定度在国际旅行卫生保健中心实验室生化检验项目中的应用。方法收集日照国际旅行卫生保健中心实验室2016年12个月的室内质控(IQC)数据和国家卫计委临床检验中心连续15次室间质评(EQA)回报结果,采用Top-Down法对血清丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)、γ-谷氨酰基转移酶(GGT)、葡萄糖(GLU)、甘油三酯(TG)和总胆固醇(TC)6项生化指标进行测量不确定度评定。结果采用深圳迈瑞公司BS-300全自动生化分析仪检测ALT、AST、GGT、GLU、TG和TC 6项生化指标合成相对扩展标准不确定度(U(%))分别为15.70%、13.46%、10.90%、6.95%、8.35%和6.82%,均小于WS/T 403-2012要求的目标扩展不确定度。结论 Top-Down法评定测量不确定度适合在国际旅行卫生保健中心实验室生化检验项目中应用。
Objective To investigate the application of top-down method to assess the uncertainty of measurement in laboratory biochemical test of International Travel Health Care Center. Methods The IQC data of 12 months in 2016 from Rizhao International Travel Health Care Center Laboratory and the results of 15 consecutive EQA returns from clinical testing center of National Health and Welfare Commission were collected and analyzed by Top-Down method Serum alanine aminotransferase (ALT), aspartate aminotransferase (AST), γ-glutamyl transferase (GGT), glucose (GLU), triglyceride (TG) and total cholesterol (TC) 6 biochemical indicators of measurement uncertainty. Results The relative standard deviations (U (%)) of biochemical indexes of ALT, AST, GGT, GLU, TG and TC detected by Shenzhen Mindray BS-300 automatic biochemical analyzer were 15.70% and 13.46% 10.90%, 6.95%, 8.35% and 6.82%, respectively, are less than the target expansion uncertainty required by WS / T 403-2012. Conclusion The measurement uncertainty of the Top-Down method is suitable for application in the laboratory biochemical test of International Travel Health Care Center.
