目的探讨重度子痫前期患者胎盘组织中胰岛素样生长因子Ⅱ(IGF-Ⅱ)以及IGFⅡmRNA结合蛋白3(IMP3)的表达情况,并分析其相关性。方法选取2014年6月-2016年12月华中科技大学同济医学院附属协和医院收治的76例重度子痫前期患者为病例组,其中40例孕28~34周早发型重度子痫前期患者为早发组,36例孕34~42周晚发型重度子痫前期患者为晚发组;另选取同一时期该院健康妊娠妇女100例为对照组,其中43例孕28~34周孕妇为早期对照组,57例孕34~42周孕妇为晚期对照组。各组胎盘组织中IGF-Ⅱ、IMP3的表达采用免疫组织化学法进行检测,对比各组胎盘组织中IGF-Ⅱ、IMP3的表达情况,并采用Spearman分析二者的相关性。结果早发组患者胎盘组织中IGF-Ⅱ、IMP3表达低于晚发组和早期对照组,晚发组患者胎盘组织中IGF-Ⅱ、IMP3表达低于晚期对照组,差异均有统计学意义(P<0.05)。Spearman秩相关结果显示,病例组胎盘组织中IGF-Ⅱ与IMP3的表达呈正相关关系(r=0.763,P<0.05)。结论不同发病期的重度子痫前期患者胎盘组织中的IGF-Ⅱ及IMP3呈不同程度的低表达,且二者呈正相关,可能参与了重度子痫前期的发病过程,联合检测有助于指导临床治疗。
Objective To investigate the expression of insulin-like growth factor Ⅱ (IGF-Ⅱ) and IGFⅡ mRNA-binding protein 3 (IMP3) in the placenta of patients with severe preeclampsia and to analyze its correlation. Methods From June 2014 to December 2016, 76 patients with severe preeclampsia who were admitted to Union Hospital of Tongji Medical College of Huazhong University of Science and Technology were selected as the case group. Among them, 40 cases of early-onset severe preeclampsia with gestational age ranging from 28 to 34 weeks were early Hair group, 36 cases of 34 to 42 weeks pregnant women with hair loss severe preeclampsia late-onset group; the other 100 cases of healthy pregnant women in the same period as the control group, of which 43 pregnant women 28 to 34 weeks as the early control group , 57 pregnant women 34 to 42 weeks pregnant women as the late control group. The expression of IGF-Ⅱ and IMP3 in placenta of each group were detected by immunohistochemical method. The expression of IGF-Ⅱ and IMP3 in placenta of each group were compared, and the correlation between them was analyzed by Spearman. Results The expression of IGF-Ⅱ and IMP3 in placenta of early-onset group was lower than that in late-onset group and early-stage control group. The expression of IGF-Ⅱ and IMP3 in placenta of late-onset group was lower than that of the late control group P <0.05). Spearman rank correlation showed that there was a positive correlation between the expression of IGF-II and IMP3 in the placenta (r = 0.763, P <0.05). Conclusions IGF-Ⅱ and IMP3 in placental tissue of patients with severe preeclampsia at different stages of development have different degrees of low expression, and the two are positively correlated, which may be involved in the pathogenesis of severe preeclampsia. Combined detection may be helpful to guide clinical treatment.
