目的分析多层螺旋CT在肺结核诊断和鉴别诊断中的应用价值。方法选取医院收治的肺结核患者75例,均行多层螺旋CT及常规X线检查,根据临床病理结果分析两种检查方法的灵敏度、特异度及准确度,同时分析CT诊断的影像学特征。结果多层螺旋CT诊断肺结核的灵敏度、准确度明显高于X线检查,差异均有统计学意义(P<0.05);两种诊断方法的特异度比较差异无统计学意义(P>0.05);多层螺旋CT诊断以磨玻璃密度与实变、树芽征、小叶间增厚为主要表现,其次是空洞、随机分布的结节、支气管及血管周围间质增厚;CT对肺内隐蔽部位病变的显示率明显高于X线检查,差异有统计学意义(P<0.05)。结论多层螺旋CT诊断肺结核的灵敏度、准确度高于常规X线检查,能真实、形象地显示肺结核的各种重要征象,因此具有较高应用价值,值得在临床推广应用。
Objective To analyze the value of multislice spiral CT in the diagnosis and differential diagnosis of pulmonary tuberculosis. Methods Totally 75 patients with tuberculosis admitted to our hospital were examined with multi-slice spiral CT and conventional X-ray. The sensitivity, specificity and accuracy of the two methods were analyzed according to the clinicopathological results. The imaging features of CT diagnosis were also analyzed. Results The sensitivity and accuracy of multislice spiral CT in diagnosing pulmonary tuberculosis were significantly higher than those of X - ray examination (P <0.05). There was no significant difference in the specificity between the two methods (P> 0.05). Multi-slice spiral CT diagnosis of ground-glass density and consolidation, tree buds, interlobular thickened as the main performance, followed by a hollow, random distribution of nodules, bronchi and perivascular interstitial thickening; CT of the hidden parts of the lung Lesion showed significantly higher than the X-ray examination, the difference was statistically significant (P <0.05). Conclusion The sensitivity and accuracy of multislice CT in diagnosis of pulmonary tuberculosis are higher than that of routine X-ray examination. It can display all kinds of important signs of tuberculosis truly and vividly, so it has high application value and is worth popularizing and applying in clinic.
