目的探讨代谢综合征(MS)各组分对2型糖尿病(T2DM)的预测价值。方法运用整群随机抽样的方法,对黑龙江省兰西县平山镇11个村屯35岁以上、≥5年的常住居民进行调查和体检,获取腰围(WC)、血压(BP)、甘油三酯(TG)、高密度脂蛋白(HDL)等数据,进行分析比较。结果排除血糖后,MS单组分中,TG、WC对T2DM有影响(TG:χ2=18.19,P<0.0001;WC:χ2=10.25,P=0.0014),MS多组分中,WC合并(BP+TG)对T2DM影响较大(χ2=24.45,P<0.0001);交互作用分析表明,WC与MS其他组分两两串联或并联时均存在正交互作用(P<0.01,S>1),对T2DM具有增效作用;灵敏度、特异度分析表明,WC合并(BP+TG)对T2DM的灵敏度、特异度均较好,正确指数、人群归因危险度百分比(PARP)、阳性似然比(+LR)最高,诊断价值较高,对T2DM有较高的预测价值。结论排除血糖后,MS单组分中TG、WC对T2DM有影响,是T2DM的预测因子,而在MS多组分中WC合并(BP+TG)时对T2DM的影响最大,预测价值最高。
Objective To investigate the predictive value of various components of metabolic syndrome (MS) on type 2 diabetes mellitus (T2DM). Methods A cluster random sampling method was used to investigate and examine the habitants of residents over 35 years old and over 5 years old in 11 villages in Pingshan Town, Lanxi County, Heilongjiang Province. Observe the changes of waist circumference (WC), blood pressure (BP), triglycerides (TG), high density lipoprotein (HDL) and other data for analysis and comparison. Results In the single component of MS, TG and WC had effects on T2DM (TG: χ2 = 18.19, P <0.0001; WC: χ2 = 10.25, P = 0.0014) + TG) had a significant effect on T2DM (χ2 = 24.45, P <0.0001). The interaction analysis showed that there was an orthogonal interaction between WC and MS other components in series or in parallel (P <0.01, S> 1) Sensitivity and specificity analysis showed that the sensitivity and specificity of WC combined with T2DM were better, the correct index, the proportion of attributable attribution of risk (PARP), the positive likelihood ratio ( + LR) highest, higher diagnostic value, higher predictive value of T2DM. Conclusion TG and WC in single component of MS can influence T2DM after excluding blood glucose, which is the predictor of T2DM. However, the effect of WC combined (BP + TG) on T2DM in MS multi-component is the greatest, and the prediction value is the highest.
